Abstract

Vasovagal and vasodepressor reactions relatively rare causes of cardiac arrest in otherwise healty patients. A 28 year old male was admitted to our hospital for minor surgical operation who had a story of syncope after intramuscular and intravenous injection. Preoperatively patient's vital signs, EKG and echocardiogram were all within normal limits. After the subcutane injection of local anaesthesic administration, almost immediately the pulse disapared. Sinus arrest and ventriculer asystole persisted for 35 seconds and were accompanied by respiratory arrest. After resuscitative efforts sinus rhythm resumed. The patient suffered no apparent sequelae from the episode of asystole and was discharged from the recovery room later that day.

Özet

Vazovagal ve vazodepressör reaksiyonlar, minör cerrahi işlemler uygulanırken, daha önce kardiyak problemi olmayan hastalarda bile, hipotansiyon, bradikardi, hatta kardiyak arreste gidebilen patolojilere sebep olabilirler. Daha önce intramuskuler ve intravenöz enjeksiyon sonrası senkop hikayesi olan 28 yaşında erkek bir hasta küçük cerrahi operasyon için hastanemize başvurdu. Preoperatif değerlendirmede, hastanın vital bulguları, EKG'si, ekokardiyogramı normal sınırlar içerisindeydi. Subkütan lokal anestezik enjeksiyonu sonrası ani kardiyak arrest ortaya çıktı. 35 saniye devam eden sinüzal arrest ve ventriküler asistoliyi respiratuar arrest takip etti. Resüsitasyonu takiben sinüzal ritm geri döndü. Hastada asistoli dönemine bağlı herhangi bir sekel ortaya çıkmaksızın bir gün sonra taburcu edildi.
