Wound infiltration via A catheter for postoperative analgesia

Abstract

We investigated the effects of local anesthetics, administered via a catheter placed in the wound, on postoperatif analgesic requirement of the subjects undergoind lower or upper extremity surgery.Fourty subjects of ASA I, aged between 20-22 were included in the study. Spinal anesthesia and supraclavicular brachial plexus blockade were performed in the lower (n=28) and upper (n=12) extremity surgery subjects, respectivelly. At the end of the surgery, a catheter was placed in teh wound and subjects were randomly allocated in two groups. 40 ml of bupivacaine 0.25 % and 40 ml of normal saline were given to Group I and II subjects, respectively. 25 mg of meperidine HCI was added if the 75 mg of diclofenac sodium and medication given via catheter was insuficient. Following the catheter treatment VAS scores were decreased in both groups, but more significantly in Group I (p< 0.05). Postoperative analgesic requirements were lower in Group I than in Group II (p< 0.05). As a result, we concluded that for preventing acute and phantom pain, wound infiltration with localk anesthetics via a catheter was reduced postoperative analgesic requirement in particularly underwent lower extremity amputation.

Özet

Alt veya üst ekstremite operasyonu geçirecek olan olgularda postoperatif analjezi sağlamak için yara içine yerleştirilen kateterden uygulanan lokal anesteziğin postoperatif analjezik gereksinimi üzerine etkileri araştırıldı. ASA l grubu, yaşları 20-22 arasında değişen 40 olgu çalışmaya dahil edildiler. Alt ekstremite operasyonu geçirecek olgularda (n=28) spinal anestezi, üst ekstremite operasyonu geçirecek olgularda (n=12) supraklaviküler pleksus blokajı uygulandı. Operasyon sonunda yara içine kateter yerleştirildi ve olgular rastgele seçimle iki gruba ayrıldılar. I. gruba 40 mi % 0.25 bupivakain. II. gruba ise 40 mi normal şalin (plasebo) verildi. Kateterden ilaç ve intramuskular 75 mg diklofenak sodyum uygulamasının yeterli olmadığı olgularda intramuskular 25 mg meperidin tedaviye eklendi.Kateterden ilaç verildikten sonra VAS skorlarında Grup l'de daha belirgin olmak üzere anlamlı azalma oldu (p< 0.05). Grup l'de postoperatif analjezik gereksinimi Grup l l'y e göre daha az oldu (p< 0.05). Sonuç olarak akut ağrı ve fantom ağrılarının gideril-mesinde, özellikle alt ekstremite amputasyonlu olgularda yara içi kateterle lokal anestezik infiltrasyonunun postoperatif analjezik gereksinimini ileri derecede azalttığı sonucuna vardık.
