A case of Whipple's disease associated with both Berger's disease and Ebstein Barr virus hepatitis

Abstract

Whipple's disease was first described in 1997 by George Hoyt Whipple. It is an uncommon multisystemic disease. The number of cases reported up to now is approximately 2000 (1). The causative agent is Tropheryma Whippleii, a small, rod shaped, intracellular gram-positive bacillus belonging to the subdivision Actinomyces (2). The symptoms at presentation are usually non-spesific and commonly due to joint involvement. Abdominal pain, malabsorption, diarrhea and weight loss may follow the presentation. As the disease advances, cachexia and involvement of the nervous system may develop. Subfebrile fever, increased skin pigmentation, peripheral tymphadenopathy, and involvement of heart, cranial nerves and central nervous system may be seen. Diagnosis can be established through histologie examination which shows Periodic Acid Schift (PAS) positive macrophages containing gram-positive, acid fast negative bacilli in biopsy specimens or molecular biological study (polimerase chain reaction (PCR)) of the involved tissues (3). Treatment is administration of antibiotics for a long period. The suggested regimen is trimethoprime / sulfamethoxazole (TMP/SMZ). Relapse can occur in patients who discontineud the therapy (4). Relapse involving central nervous system is a bad prognostic indicator (5). Here we triad to report a case of Whipple's disease associated with both acute Ebstein Barr Virus hepatitis and Berger's disease. Berger's disease has been first diagnosed by Berger and Hinglais in 1968 (6). The first case has been a patient with hepatic disease and Henoch Schönlein Purpura. Till today many cases associated with IgA nephropathy were published. In same of these reports, Whipple's disease was shown as a rare cause of Berger's disease (7) The variations in extraintestinal involvements are discussed.

Whlpple hastalığı ilk kez 1907'de George Hayt Whipple tarafından tanımlanmıştır. Nadir bir multisistemik hastalıktır. Şimdiye kadar tanımlanmış olgu sayısı yaklaşık 2000'dir (1). Etken aktinomiçet grubundan gram pozitif, intrasellüler, Tropheryma Whippelii isimli basildir (2). Hastalık, genellikle nonspesifik semptom ve eklem tutulumuyla başlar. Daha sonra; abdominal ağrı, malabsorpsiyon, diyare, kilo kaybı, geç dönemde ise sinir sistemi tutulumu ve kaşeksl ile karakterizedir. Subfebril ateş, deri pigmentasyonunda artma, periferik lenfadenopati, santral sinir sistemi tutulumu, kraniyal sinir tutulumu ve kardiyak tutulum gözlenebilir. Tanı, intestinal ve/veya diğer dokulardan yapılan biyopside Periodik Asit Schift (PAS) pozitif boyanan glikoprotein içeren köpüksü makrofajların görülmesi ve polimeraz zincir reaksiyonu (PCR) ile konulur (3). Tedavi uzun süreli antibiyoterapidir. Pek çok tedavi kombinasyonları kullanılsa da önerilen tedavi trimetoprin-sulfamektoksazoldur (TMP-SMZ). Antibiyotik tedavisi kesilen hastalarda relaps görülebilir (4). Santral sinir sistemi relapsı kötü prognoz gösterge-sidir (5). Bu çalışmada akut EBV (Ebstein Barr Virüs) Hepatiti ve Berger Hastalığı ile birlikte olan bir olgu sunulmuştur. Berger Hastalığı ilk kez 1968 yılında Berger ve Hinglais tarafından tanımlanmıştır (6). ilk olgu karaciğer hastalığı ve Henoch Schönlein purpurası olan bir hasta olarak bildirilmişti, ilerleyen yıllarda IgA nefropatisine eşlik eden birçok hastalık tanımlanmıştır. Bunlar arasında ender rapor edilmekle birlikte, Whipple hastalığı da vardır (7). Makalede ayrıca barsak dışı tutulumların değişik formları ele alınıp tartışıldı.
