Management of midface fractures with early rigid fixation and primary bone grafting

Abstract

Between 1993 and 1996, extended open reduction and rigid fixation and primary bone grafting if needed were employed in 110 patients with midface frac-tures at GATA Plastic and Reconstructive Surgery Department. Open reduction of the condyle was also employed in two Le Fort fracture patients with loss of ramus height. The orbital floor defects in 23 patientswere reconstructed by using biomaterials (porous polyethylene implant, titanium mesh) in addition to autologous bone grafts. Experience with 110 midface fractures in multiple trauma patients has shown that superior aesthetic and functional results may be obtained by immediate extended open reduction with primary bone grafting. Structural bony integrity and preinjury facial architecture may be restored in the absence of soft tissue contracture.

1993-1996 yıllarında GATA Plastik ve Rekonstrükitf Cerrahi Kliniğinde orta yüz yöresinde kırık saptanan toplam 110 hastaya geniş açık redüksiyon, rijid internal fiksasyon ve gerektiğinde primer kemik grefti ile onarım uygulandı. Le Fort kırığına eşlik eden kondil kırığı nedeniyle iki olguda, kondile yönelik açık redüksiyon uygulandı. Orbita taban defekti saptanan 23 olgunun defektleri otojen kemik greftlerine ek olarak biyomateryaller (poröz polietilen implant, titanyum mesh) kullanılarak onarıldı. Orta yüz kırığı bulunan 110 travmalı hastadaki deneyimlerimiz, en iyi estetik ve fonksiyonel sonuçların erken açık redük-siyon ve gerekli olgularda primer kemik grefti kullanımı ile mümkün olabileceğini göstermiştir. Yapısal kemik bütünlüğü ve yaralanma öncesi yüz yapısı ancak yumuşak doku kontraksiyonu gelişmeden sağlanabilir.
