Assessment of intravascular fluid volume and dry weight by non-invasive techniques in chronic hemodialysis patients

Abstract

Correct estimation of dry weight remains a major problem in the clinical practice of hemodialysis. Over-estimation of dry weight leads to chronic, although not always fluid overload, resulting in hypertension and cardiac dilatation, while under-estimation leads to problems related to underhydration and hypotension. Therefore, several new techniques to assess dry weight have been developed recently. In this research, the uses of clinical, imaging and biochemical methods for determination of dry weight and intravascular fluid volume in hemodialysis patients were evaluated in three studies. In the first step of study the fluid status was measured by cardiothoracic index (CTI), mean arterial pressure (MAP), body mass index (BMI), vena cava inferior diameter (VCI-D) in 28 chronic hemodialysis (CHD) patients before (BD) and after dialysis (AD) and in healthy subjects. Collapse Index (CI) and vena cava index (VCI), the so-called inferior vena cava indices, were calculated by using VCI-D measurements. In the second step, according to interdialytic weight gain (IDWG), patients on dialysis were sererated in two groups (Group-1, n:14,IDWG >2.5 kg: Group-2, n: 14, IDWG<2.5 kg)and fluid status was determined BD and AD by the parameters mentioned above. In the third step according to MAP , patients on dialysis were seperated in two groups (Group -H, n:8, MAP>108 mmHg; Group-N, n:20, MAP <108 mmHg) and fluid status was determined BD ad AD by the parameters mentioned above.CHD patients had significantly greater CTI,MAP, VCI-D than the control BD (p<0.05) When compared the values of the patients BD and AD, all differences of parameters were found significant (p<0.05) but the means of BMI (p>0.05) In the second step, Group-1 patients had significantly greater CTI (p<0.001) than group-2 patients . When compared the values of the patients BD and AD, all differences of parameters were found insignificant (p> 0,05) but the means of BMI (p<0.05) In the third step group-H patients had significantly greater CTI (p<0.001) than Grıup-N patients when there was no significant difference between their IDWG values (p>0.05) When compared the values of the patients BD and AD, all differences of parameters were found significant (p<0.05) but the means of BMI (p>0.05) We conclude that the non-invasive techniques such as measurements of CTI VCI-D are appropriate methods for dry weight and intravascular fluid volume, besides MAP; hypertension is a more accurate indicator of fluid retention than IDWG anf their is a chronic volume overload in hypertensive patients on hemodialysis, even after dialysis.

Kuru ağırlığın doğru öngörülmesi hemodiyaliz uygulamasında önemli bir sorundur. Kuru ağırlığın olduğundan daha yüksek öngörülmesi, her zaman belirgin olmasa da, kronik sıvı yüklenmesine, hipertansiyon (HT) ve kalp büyümesine neden olurken; olduğundan daha düşük öngörülmesi dehidratasyon ve hipotansiyon ile birlikte gelişen sorunlara yol açmaktadır. Bu nedenlerden ötürü kuru ağırlığın belirlenmesi için son zamanlarda yeni yöntemler geliştirilmiştir. Bu çalışmada kronik hemodiyaliz (KHD) hastalarında klinik, radyolojik ve biyokimyasal yöntemler kullanılarak kuru ağırlık ve intravasküler sıvı hacminin durumu araştırılmıştır. Çalışmanın birinci aşamasında, diyaliz öncesi (DO) ve diyaliz sonrası (DS) dönemde 28 KHD hastası ile sağlıklı kontrol grubu bireylerinin kardiyotorasik indeksi (KTl), ortalama arter basıncı (OAB), beden kitle endeksi (BKl), inferior vena kava çapı (IVK-Ç) değerleri belirlenmiştir. IVK-Ç ölçümlerinden yararlanılarak, !VK indeksleri olarak adlandırılan kollaps indeksi (Ki) ve vena kava indeksi (VKl) hesaplanmıştır. Çalışmanın ikinci aşamasında KHD hastaları interdiyalitik ağırlık artışına (IDAA) göre iki gruba ayrılmış (Grup-1, n : 14, IDAA > 2.5 kg.; Grup-2, n : U, IDAA 108 mmHg; Grup-N, n : 20, OAB 0.05), diğer tüm parametreler arasındaki farkların anlamlı olduğu görülmüştür (p 0.05) belirlenmiştir. Grup-H ve Grup-N arasında VKl bakımından gerek DO ve gerekse DS ölçümlerde farklılık bulunmuş (p 0.05). OAB yanında KTl, IVK-Ç ölçümleri gibi non invazif tekniklerin kuru ağırlık ve intravasküler sıvı hacminin belirlenmesi için uygun yöntemler ve HT'un iDAA'na göre daha güvenilir bir sıvı retansiyonu belirteci olduğu, hipertansif KHD hastalarında diyaliz sonrasında da devam eden kronik bir hacim yüklenmesi bulunduğu sonuçlarına varılmıştır.
