Fetal ultrasound abnormalities in 108 pregnancies: Correlation with fetal karyotype, autopsy findings, radiological findings and postnatal outcome

Abstract
The study included 108 pregnancies who were diagnosed to have fetal anomalies during routine ultrasonographic examination between 14-38 weeks of gestation. Cases were evaluated according to the ultrasonographic, cytogenetic, pathological and radiological findings as 'well as outcomes of gestation. Cytogenetic studies were conducted by amniocentesis, cordocentesis, chorionic villus, cardiac blood, and skin biopsy of 107 out of 108 cases. In one case, Meckel-Gruber syndrome, cytogenetic study could not be conducted because of inappropriate material and the case was diagnosed clinically and pathologically. Twelve chromosomal abnormality was found out of these 107 (11.21 %) cases studied: three cases of+ 18, three cases of+ 21, one case of mosaic + 21, two cases of monosomy X, one case of 46, XX, t(17;22)(p 11.2; p 13) pat, one case of t(16) (pter $\rightarrow$ p 11.2 :: q 24$\rightarrow$ qter). Major sonographical findings of trisomy 18 cases were "polyhydramnios", "hydrocephaly", "choroid plexus cyst" anf "IUGR"; of trisomy 21 cases were "isolated hydrocephalus", "polyhydramnios" and "cystic hygromaand hydrops fetalis"; of monosomy X were "hydrops fetalis" and "cystic higroma"; of t(17;22) (p 11,2;p13)pat was "occipital encephalocel"; of t(16)(pter$\rightarrow$p 11.2::q24 $\rightarrow$ qter)was "choroid plexus cyst"; of triploidy case was increased "nuchal translucency " and of mosaic trisomy 21 case was "intrauterine growth retardation" Among 95 cases with no chromosomal abnormality, 7 were grouped as presenting a monogenic syndrome , 21 as a multifactorial inheritance, 6 as an amniotic fluid volume abnormality and 27 as normal 32 cases presented congenital malformations of unknown etiology. This study shows the importance of multidisciplinary approach for the evaluation of sonographically detected fetal abnormalities.

Çalışma, 14-38. gebelik haftalarında yapılan rutin ultrasonografik incelemede fetal anomali tanısı konan 108 gebeliği kapsamaktadır. Olgular, ultrasonografik, sitogenetik, patolojik ve radyolojik bulgular ile postnatal sonuçları ele alınarak değerlendirilmiştir. 108 olgudan 107'sinde, amniyosentez, kordosentez, koryonik doku, kardiyak kan ya da cilt biopsisi ile sitogenetik çalışma yapılmıştır. Meckel-Gruber sendromlu bir olguda, materyalin uygun olmaması nedeni ile sitogenetik inceleme yapılamamış, tanı klinik ve patolojik olarak konmuştur, incelenen bu 107 olgudan 12'sinde (% 11.21) sitogenetic anomali gözlenmiştir. Trizomi 18 (bir olguda), trizomi 21 (3 olguda), mozaik trizomi 21 (bir olguda) monozomi X (iki olguda), t(17; 22) (p 11.2; p 13) pat sitogenetik anomalisi (bir olguda), t(16) (pter $\rightarrow$ p11.2 :: q24 $\rightarrow$ qter) sitogenetik anomalisi (bir olguda) gözlenen anomalilerdir. Trizomi 18 gözlenen olgularda; polihidroamniyos, hidrosefali, koroid pleksus kisti ve intrauterin gelişme geriliği, trizomi 21 gözlenen olgularda; izole hidrosefali, polihidroamniyos, kistik higroma ve hidrops fetalis, monozomi X gözlenen olgularda;hidrops fetalis ve kistik higroma, t(17; 22)(p 11.2; p 13) pat gözlene olguda; oksipital ensefelosel, t(16) (pter$\rightarrow$ p 11.2: q24$\rightarrow$ qter) gözlenen olguda; koroid pleksus kisti, triploidi olgusunda; ense kalınlığında artma, mozaik trizomi olgusunda; intrauterin gelişme geriliği major ultrason bulguları olarak saptanmıştır. Kromozomal anomali saptanmayan 95 olgudan 7'si monogenik sendrom 21'i multifaktöryel kalıtım, 6'sı amniyotik sıvı hacim anomalisi, 2'siplasental anomali ve 27'si normal olarak sınıflandırılmıştır. Otuziki olgu etyolojisi bilinmeyen konjenital malformasyon olarak değerlendirilmiştir. Bu çalışma ultrasonografik olarak saptanan fetal anomalilerin multidisipliner olarak çalışılmasının önemini göstermektedir.
