Comparison of the techniques balanced anesthesia and TIVA+N2O monitoring

Abstract

Somato-sensory evoked potantials (SSEP) monitoring was carried out in patients underwent scoliosis surgery for preventing neural damage. It is known that SSEP monitoring is effected by various factors. It was purposed to investigate whether the effects on SSEP monitoring and superiorities themselves of TIVA and balanced anaesthesia methods carried-out in two different patient groups in this study. Sixty-four ASA I-II patients, age 11-64 were included between 1994-1997 years in the study. Following the premedication of 10 mg diazepam oral and 0.5 mg atropine i.m., patients were allocated at random in two groups. At induction 4-6 mg/kg of sodium thiopental and 2 mg/kg propofol were given in Group I and Group II respectively. For analgesia 0.05-0.1 mg of fentanyl and for facilitating the intubation 0.1 mg/kg vecuronium bromide were given in both groups. For the maintenance of anaesthesia, isoflurane 1-1.5% and .i.v. propofol 9 mg/kg/hour were administered in Group I and II respectively with N2O 60%+O2 40%. Vecuronium was repeated as required. Non-invasive monitorization was performed (MAP, ECG, EtCO2, SpO2, temperature and heart rate) in all patients and SSEP monitorization was performed by using Dantec Evolution (Software 1.66 Version-1992) preoperatively, intraoperatively and postoperatively. A false and a real positive result were obtained in Group I and Group II respectively. In all the other patient right results were obtained. No significant difference between groups was detected in the monitored parameters. As a result it was concluded that there was no superiority between compared two methods and both of them will be used safely.

Skolyoz operasyonu uygulanan olgularda olası nöral hasarın önlenebilmesi amacı ile Somato-sensoryal evok potansiyellerin (SSEP) monitörizasyonu yapılmaktadır. SSEP monitörizasyonunun değişik etmenlerden etkilendiği bilinmektedir. Çalışmada iki ayrı hasta grubunda uygulanan TİVA ve dengeli anestezi yöntemlerinin SSEP monitörizasyonuna etkilerinin ve birbirlerine üstünlüklerinin olup olmadığının araştırılması amaçlandı. Yaşları 11 ile 64 arasında değişen ASA I-II grubuna giren 64 olgu çalışmaya alındı. Operasyondan yaklaşık 1 saat önce 10 mg oral diazepam ve 0.5 mg i.m. atropin uygulanan olgular rastgele seçimle iki gruba ayrıldılar. Genel anestezi indüksiyonunda Grup I olgularda İ.V.yol ile (n=33) 4-6 mg/kg sodyum tiyopental, Grup II olgularda (n=31) 2.5 mg/kg propofol, analjezik olarak her iki grupta 0.05-0.1 mg fentanil ve kas gevşetici olarak 0.1 mg/kg ve kuronyum bromid kullanıldı. Anestezi uygulamasının sürdürülmesinde %60 N2O + % 40 O2 solutulması ile birlikte Grup I'de % 1-1.5 izofluran, Grup II'de ise İ.V. 9 mg/kg/saat propofol kullanıldı. Gerektiğinde ve kuronyum bromid dozları tekrar edildi. Olguları izlemek için noninvaziv monitorizasyon uygulandı. (TA, EKG, SpO2 ve EtCO2 , vücut ısısı, nabız sayısı). Dantec Evolution (Software 1.66 Version-1992) kullanılarak olguların tümüne preoperatif, intraoperatif ve postoperatif SEP yapıldı. Grup I'de 1 yanlış pozitif sonuç, Grup II'de 1 gerçek pozitif sonuç elde edildi. Diğer olguların tümünde doğru ölçümler elde edildi. Olguların izlenen parametrelerinde gruplar arası belirgin farklılıklar saptanmadı. Sonuç olarak; karşılaştırılan yöntemlerin birbirlerine üstünlükleri olmadığı ve her iki yönteminde güvenle kullanılabileceği kanısına varıldı.
