The effect of the procedure that was prepared for medication administrations on medication errors

Abstract

Thinking of recent developments an effective and reliable medication administration is perceived as a process that is provided with multidisipliner collaboration and communication. This process begins with the producer that produces the drug and continues with doctor, pharmacist and nurse and comes to an end with the patient. Errors may occur in any stage of this process. The purpose of this research is to determine the medication administration errors in the application process system, to practice the procedure that was prepared for medication administration and to determine the effect of this procedure on medication errors. It was planned as descriptive and comparative. This research was done in physical therapy and rehabilitation clinic in Gülhane Military Medical Academy Hospital. The universe of this research consists of the medication administrations on practice times during one year period while medication administrations during totally 80 practice times in January and March 1998 are included in the sample. Before using the procedure, the administrations in practice times were the control group (The administrations during 40 practice times in January). After using the procedure, the administrations in practice times were the experimental group (The administrations during 40 practice times in February). The data were collected by the "Medication Errors Determination Form" by observing before and after the administration. For analyzing the data that were collected, percentages, shi-square, Fisher's definite chi-square test and the importance test of difference between two aver-ages were used. According to the results of this research, we can say that: During the stages of taking order, preparing and administering medicines, the average times before and after using the procedure were found statistically significant. Types of errors, factors causing errors and stages occurred errors and error occurrence stages before and after using the procedure were found statistically significant. It was evaluated that this procedure that were found as a positive contributor to medication administration process and that was prepared for this study should be reorganized according to the changeable conditions. Also, the continuity of this procedure in practice should be provided with.

Özet

Güncel eğilimler etkin ve güvenilir ilaç uygulamalarını disiplinler arası işbirliği ve iletişimle sağlanabilen bir süreç olarak görmektedir. Bu süreç, ilaçlan üreten üreticiyle başlayıp, hekim, eczacı, hemşire ile devam etmekte ve ilacı alan hastayla sonlanmaktadır. Hatalar, bu sürecin herhangi bir basamağında meydana gelebilir. Bu araştırma, ilaç uygulama süreci içinde var olan sistemdeki İlaç hatalarını tespit etmek, ilaç uygulamalarına ilişkin geliştirilen işlemi uygulamak ve uygulanan bu işlemin ilaç hatalarına etkisini saptamak amacıyla tanımlayıcı ve karşılaştırmalı olarak planlanmıştır. Araştırma GATA Eğitim Hastanesi FTR kliniğinde yapılmıştır. Araştırmanın evrenini FTR kliniğinde, bir yıl içinde ilaç saatlerinde gerçekleşen ilaç uygulamaları, örneklemini ise 1998 Ocak ve Mart aylarında gerçekleşen toplam 80 ilaç verme saatindeki uygulamalar oluşturmuştur. Geliştirilen işlemi uygulamadan önce gözlenen ilaç saatindeki uygulamalar (Ocak ayı içinde 40 ilaç verme saatinde gözlenen uygulamalar) kontrol grubunu, prosedür uygulandıktan sonra gözlenen ilaç saatlerindeki uygulamalar (Şubat ayı içinde 40 ilaç verme saatinde gözlenen uygulamalar) ise deney grubunu oluşturmuştur. Araştırmadaki veriler "İlaç Hataları Tespit Formu" ile geliştirilen işlem uygulanmadan önce ve uygulandıktan sonra gözlem yapılarak toplanmıştır. Elde edilen verilerin değerlendirilmesinde yüzdelik sayılar, ki-kare, Fisher's kesin ki-kare testi ile iki ortalama arasındaki farkın önemlilik testi kullanılmıştır. Araştırmadan elde edilen başlıca sonuçlar şunlardır: İstemin alınması, ilaçların hazırlanması ve uygulanması safhalarında işlemin uygulanması öncesi ve sonrasında harcanan süre ortalamaları açısından istatistiksel olarak anlamlı bir fark bulunmuştur. Hata tipleri, hataya neden olan faktörler, hatanın oluştuğu safhalar açısından uygulama öncesi ve sonrasında istatistiksel olarak anlamlı bir fark bulunmuştur ve hataların büyük oranda azaldığı saptanmıştır. İlaç uygulama sürecine olumlu yönde katkı sağladığı saptanmış olan ve bu çalışma kapsamında geliştirilen "İlaç Uygulamalarına İlişkin İşlem"in, değişen şartlara göre yeniden düzenlenerek uygulamada sürekliliğin sağlanmasının faydalı olacağı değerlendirilmektedir.
