The role of effective control of acute emesis and comparison of dexamethasone with ondansetron plus dexamethasone in the control of cisplatin-induced delayed emesis

Abstract

The role of the 5- hydroxytriptamine receptor antagonists in the control of delayed emesis is not yet clear. A total of 81 chemotherapy-naive patients (of whom 75 were evaluable) entered this study to compare the efficacy and tolerability of dexamethasone (DEX) and ondansetron (OND) plus DEX for the prevention of cisplatin induced delayed emesis. All patients received the same antiemetic treatment for acute emesis. On day 2 (24 hours after cisplatin administration, patients were randomized to receive DEX (8 mg im bid on day 2-3 and 4 mg im bid on day 4-5) or OND (8 mg orally bid on day 2-5) plus DEX (as above). Patient characteristics were well balanced between the two treatments. Over days 2-5 the complete response rates were 58.9% with DEX and 61.1% with OND plus DEX (no significant difference). Significantly patients obtaining complete protection from acute emesis achieved a high rate of complete protection from delayed emesis (67.8% vs 37.5% with OND plus DEX, p< 0.05; 66.6% vs 33.3% with DEX, p<0.05). The antiemetic treatments were similarly well tolerated. The most frequent adverse events were constipation and headache. There were no significant differences in the safety profiles of the two regimens with the exception of constipation that was more frequent in the OND plus DEX group. In conclusion, the combination of OND and DEX did not improve significantly on the control of delayed emesis achieved using DEX alone. Our results confirm that protection from acute emesis plays a major role in the control of delayed emesis.

Özet

Gecikmiş emezis kontrolünde 5-hidroksitriptamin reseptör antagonistlerinin rolü belirgin değildir. Bu çalışmaya sisplatine bağlı gecikmiş emezisin önlenmesinde dekzametazon (DEX) ile ondansetron (OND) artı DEX kombinasyonunun etkinliğini ve tolerabilitesini karşılaştırmak için daha önce kemoterapi almamış total 81 hasta (bunların 75'i değerlendirilebilir idi) alınmıştır. Tüm hastalar akut emezis için aynı antiemetik tedavi almışlardır. Sisplatin verilmesinden 24 saat sonra (2. gün) hastalar DEX (2-3. günler günde 2 kez im 8 mg, 4-5. günler günde 2 kez im 4 mg), yada OND (2-5. günler günde 2 kez oral 8 mg) artı DEX (aynı şekilde) olarak randomize edilmişlerdir. Kemoterapinin 2-5. günleri kombine değerlendirildiğinde tam yanıt oranları DEX ile %58.9 ve OND artı DEX ile %61.1 idi (anlamlı fark yoktu). Akut emezis fazında tam yanıt elde edilen hastalarda gecikmiş emezis fazında anlamlı olarak daha fazla tam yanıt elde edilmiştir (OND artı DEX grubunda %67.8'e karşı %37.5, p<0.05; DEX grubunda %66.6'a karşı %33.3, p<0.05). Antiemetik tedaviler iyi tolere edilmiştir. En sık görülen yan etkiler konstipasyon ve baş ağrısı idi. OND artı DEX grubunda daha fazla olan konstipasyon dışında yan etkiler bakımından iki rejim arasında anlamlı farklar yoktu. Sonuç olarak OND artı DEX kombinasyonu yalnız DEX kullanımından anlamlı olarak daha fazla etkinlik sağlamamıştır. Ayrıca sonuçlarımız akut emezisin kontrolünün gecikmiş emezis kontrolünde büyük bir rol oynadığını doğrulamaktadır.
