Transperineal bulboprostatic anoastomatic urethroplasty for complex posterior urethral stricture

Abstract
Transperineal bulbo-prostatic anostomotic urethro-plasty(TBPAU) is the surgical procedure of choice dealing with the longly obliterated urethra or urethral 'stricture' with a pelvic fracture due to injury to the bul-bomembranous urethra with complete loss of urethral continuity. Following endoscopic uretroplastic procedures, TBPAU may be the more succesful method from the other urethroplasties for reconstruction. The procedure depends on a progressive series of steps to capitalise on the elastisity of the urethra and then straigthen out the nature curve of bulber urethra together with intrapubic penil relaxation or inferior pubectomy if needed. 8 patients with complicated urethral stricture underwent TBPAO. By additional endoscopic methods in the other management steps, all the patients mictuate through their native urethra with having continence
Özet
Transperineal bulboprostatik anastomatik üretroplasti (TBPAU)pelvis kırıkları ile olan komplike membranoz üretral darlıklar veya üretral devamlılığın uzun bir segment ile kesintiye uğradığı ciddi yaralanmalar sonrası bir seçenek olabilir. Endoskopik üretroplasti teknikleri kullanılmaya çalışıldıktan sonra başarı elde edilememişse, alternatif üretroplasti teknikleri yerine TBPAU ile yapılacak bir rekonstrüksiyonun başarısı daha yüksek olabilir. Teknik anterior üretranın olabildiğince serbest/eştirilerek bulboz üratranın doğal eğiminin tamamen düzleştirilmesini ve gerektiğinde inferior pubektomiyi ifade eder. Bu tekniki kullanarak 8 komplike posterior üretral darlığı öpere edildi ve ek olarak endoskopi teknikleri kullanarak hepsi üretralanndan işer ve idrarını tutabilir konuma getirildi.
