The effect of the scolocidal agents on the liver and bile ducts

Abstract
In this study the effect of the scolocidal agents, which are mostly preferred in the management of the hydatid disease of the liver, on the liver and bile ducts was investigated. For this purpose, after examining the liver function tests and serum alkaline phos¬phatase, the gallbladder was catheterized via laparo¬tomy in 30 rabbits. The laboratory animals were divid¬ed into 6 groups, 5 in each as control group, absolute alcohol group, % 10 PVP-i group, % 5 NaCI group, % 10 NaCI group, % 20 NaCI group. After the common bile duct was clamped, the gallbladder was infused with 6 different scolocidal agents and we waited for 5 minutes in all animals. The laparotomy was complet¬ed after suturing the infusion site, and the animals were closely followed-up for 30 days. The blood sam¬ples were again obtained for the liver function tests and serum alkaline phophatase at the 30th day. The tissue specimens of the liver, bile ducts and gall¬bladder were removed by sacrification. As a result; when the liver function tests and serum alkaline phosphatase levels after the infusion of scolocidal agents were compared with the levels before the infusion, there was no statistically significant differ¬ence. However, in the groups in which absolute alco¬hol and hypertonic sodium chloride solutions (10%, 20%) were used, congestion, balloning degeneration, periductal inflammation, focal necrosis in the parenchyma of the liver and bile ducts, proliferation, fibrosis, precirrhotic changes in the bile ducts and the stasis of the bile were observed clearly more than the other groups. The histopathological changes caused by the absolute alcohol and hypertonic sodium chlo¬ride solutions (10%, 20%) on the liver and the bile ducts may develop even in such a short time as one month. In conclusion, we suggest that absolute alco¬hol and hypertonic sodium chloride 10 - 20% solu¬tions shouldn't be used in the hydatid disease of the liver with possible communication to the bile ducts.

Bu çalışmada karaciğer kist hidatik cerrahi tedavisinde en fazla tercih edilen skolosidal mad¬delerin karaciğer ve safra yollarına etkileri araştırılmıştır. Bu amaçla 30 adet tavşanda alkalen fosfataz ve karaciğer fonksiyon testleri çalışıldıktan sonra laparotomi yapılarak safra kesesi kateterize edilmiştir. Deney hayvanları kontrol grubu, absolü alkol grubu, % 10 polyvinylpyrolidone iod(PVP-i) grubu, % 5 NaCI grubu, % 10 NaCI grubu ve % 20 NaCI grubu olarak 5'erll 6 gruba ayrılmıştır. Tüm hay¬vanlarda koledok klempe halde iken 6 farklı skolosi¬dal madde safra kesesi içine enjekte edilerek 5 daki¬ka beklenmiştir. Enjeksiyon yeri sütüre edilerek laparotomi sonlandırılmıştır. 30 gün sonunda alkalen fosfataz ve karaciğer fonksiyon testleri için tekrar kan örneği alınmıştır. Sakrifikasyondan sonra karaciğer, safra kesesi ve safra yollarından doku örnekleri alın¬mıştır. Çalışma sonrası karaciğer fonksiyon testleri ve alkalen fosfataz değerlerinde çalışma öncesine göre istatistiksel olarak anlamlı bir değişiklik gözlen¬memiştir. Histopatolojik olarak absolü alkol, %10 NaCI ve %20 NaCI gruplarında; karaciğer paranki-minde presirotik değişiklikler önemli konjesyon, bulanık şişme, periduktal inflamasyon, fokal nekroz ile safra yollarında proliferasyon, fıbrozis ve safra stazı diğer gruplara göre belirgin derecede daha fazla gözlenmiştir. Hipertonik NaCI ve absolü alkolün safra yolları ve karaciğerde oluşturduğu histopatolojik değişiklikler 1 ay gibi kısa bir sürede dahi ortaya çıka¬bilmektedir. Bu bulgulara dayanarak kanımızca safra yolları ile irtibatlı olabilecek karaciğer kist hidatik-lerinde absolü alkol ve % 10 - 20'lik hipertonik NaCI kullanılmamalıdır.
