Long-term central venous catheter insertions for bone marrow transplantation

Abstract

It is required to insert a long-term central venous catheter for bone marrow transplantation. Purpose: We evaluated insertion technique, early and late complications relating with the long-term central venous catheter in this immune compromised patient group. Material and Method: During a period between January 1st. 1988 and December 31st. 1999, long-term central venous catheter has been inserted to 135 patients with hematological diseases or solid tumors and planned for either allogenic or autologous bone marrow transplantation. We evaluated all patients having inserted catheter for early and late period complications by either radiological or microbiologic measures. White blood cell or irombocyte have been counted before or after catheter insertion whenever it is required. We recorded the period of catheter in use. We cultured the tip of the catheter during catheter withdrawal. Results: We inserted 135 long-term central catheters for bone marrow transplantation in 82 patients with solid tumors and totally 53 patients with hematological diseases, 9 with benign and 44 with malign disease. 44 of those were female, 91 were male. The average age was 31 ± 11,6 years old, ranging between 15 and 76.Catheter insertion has been performed via internal jugular venotomy in 8 patients and via subclavian vein using Seldinger technique in 127 patients. Average catheter usage was 44 days, minimum seven and maximum 360 days. We have seen early complications in nine and late complications in 20 patients. Conclusion: It is the most logical way to insert a long term central venous catheter for bone marrow transplantation because it prevents two steps procedure. The insertion of long term central venous catheter in immune compromised patients for bone marrow transplantation can be used with a reasonable risk rate.

Kemik iliği transplantasyonu için uzun süreli santral kateter uygulaması gerekmektedir. Amaç: İmmun sistemi baskılanan bu hasta grubunda uzun süreli santral katetere bağlı erken ve geç dönem komplikasyonlar ile girişim tekniklerini değerlendirdik. Gereç ve Yöntem: 1 Ocak 1988 ve 31 Aralık 1999 arası dönemde allojenik veya otolog kemik iliği transplantasyonu planlanan hematolojik hastalığı veya solid tümörü olan 135 hastaya uzun süreli santral kateter uygulanmıştır. Kateter takılan tüm hastalar radyolojik ve mikrobiyolojik tetkiklerle erken ve geç dönem komplikasyonlar için değerlendirilmiştir. Kateter uygulanması öncesinde ya da sonrasında gerekli olduğunda beyaz küre ve trombosit sayımı yapıldı. Kateterin kullanıldığı süreyi kaydettik. Tüm kateterler çekilirken kateter ucu kültürü aldık. Bulgular: Kemik iliği transplantasyonu için 135 uzun süreli santral kateteri 9'u benign ve 44'ü malign olmak üzere toplam 53 hematolojik hastalığı olan ve 82 solid tümörlü olguya uyguladık. Hastaların 44'ü kadın, 91'i erkek idi. Yaş ortalaması 37±11,6 yıl, yaş aralığı 15 - 76 yıl idi. Kateter 8 olguda internal juguler venotomi ile 127 olguda subklavian vene Seldinger tekniği ile yerleştirildi.Ortalama kateter kullanım süresi 44 gün olup, en kısa 7 en uzun 360 gün süreyle kullanıldı. 9 olguda erken dönem, 20 olguda geç dönem komplikasyon gördük. Sonuç: Kemik iliği transplantasyonu için iki etaplı işlemi ortadan kaldırmasıyla uzun süreli santral kateter uygulaması son derece akılcı bir yöntemdir. İmmün sistemi baskılanmış kemik iliği transplantasyonlu hastalarda uzun süreli santral kateter uygulaması kabul edilebilir risk oranlarıyla yapılmaktadır.
