The childhood traumatic events and psychiatric disorders in patients with posttraumatic stress disorder

Abstract

Objective: It has been reported that traumatic events during childhood have been associated with adult psychopathology. The purpose of the study was to examine the frequency of childhood traumas and its associations with posttraumatic stress disorder (PTSD) symptoms. Method: Forty seven male patients were diagnosed as PTSD by using Structured Clinician Interview for DSM-III-R Diagnosis (SCID). Childhood abuse, conduct problems, enuresis, school phobia, speech problems, nail biting, and tics were evaluated with the Childhood Traumatic Events Questionnaire Form. The severity of PTSD symptoms was assessed with the Clinician-Administered PTSD Scale 1-2 (CAPS 1-2) and the Impact of Event Scale (IES). The depression and anxiety levels were measured by the Beck Depression Inventory and the State-Trait Anxiety Inventory I-II (STAI I-II). The relationship between childhood abuse and PTSD symptoms was analyzed. Results: Childhood traumatic events were found in 70.2 % of patients. There were neglect in 57.4%, physical abuse in 44.7%, emotional abuse in 31.9%, and sexual abuse in 8.5% of the patients. Other childhood problems were conduct problems (55.3%), enuresis (34%), nail biting (34%), school phobia (31.9%), speech problems (25.5%), and tics (19.1%). The severity and frequency of avoidance and hyperarousal symptoms, social, and occupational dysfunctions were significantly higher among patients with history of trauma than those without (p<.05). Nevertheless, the severity of intrusive symptoms was not significantly different (p>.05). Conclusion: These results indicate that childhood trauma and the other problems are the quite common in patients with PTSD. These unpleasant events may be predictive for the severity of PTSD.

Amaç: Çocukluktaki travmatik olayların psikiyatrik bozukluklarla ilişkili olduğu bilinmektedir. Çalışmada, travma sonrası stres bozukluğu (TSSB) nda çocukluk dönemi travmatik yaşam olayları ile TSSB semptomları arasındaki ilişki araştırılmıştır. Yöntem: DSM-III R'nin Klinisyen için Yapılandırılmış Klinik Görüşme Formu (SCID) ile TSSB tanısı konulan 47 erkek hastada; Çocukluk Çağı Travmatik Yaşantıları Sorgulama Formu ile çocukluk çağı travmatik yaşantıları ve davran im sorunları, Klinisyen için TSSB Ölçeği 1-2 (CAPS 1-2) ve Yaşanan Olayın Etkisi Ölçeği (IES) kullanılarak TSSB semptomlarının yoğunluğu, Beck Depresyon ve Durumsal ve Sürekli Anksiyete Ölçeği I-II (STAI I-II) ile depresyon ve anksiyete düzeyleri ölçülmüştür. Bulgular, sıklık ve birbirleri ile ilişkisi açısından analiz edilmiştir Bulgular: Hastaların %57.4'ünde ihmal edilme, %44.7'sinde fiziksel, %31.9'unda emosyonel ve %8.5'inde cinsel kötüye kullanılma olmak üzere toplam %70.2'sinin çocukluğunda travmatik yaşantı, %55.3'ünde en az bir davranım bozukluğu belirtisi, %34'ünde enürezis noktuma, %34'ünde tırnak yeme, %31.9'unda okul korkusu, %25.5'inde kekemelik ve %19.1'inde tik bozukluğu belirtilmiştir. Çocukluk çağı travmatik yaşantısı belirten hastalarda kaçınma ve artmış uyarılmıştık semptomlarının, sosyal ve mesleki işlevsellikteki bozulmanın şiddeti, travma öyküsü olmayanlara oranla anlamlı düzeyde farklı bulunurken (p .05) saptanmamıştır. Sonuç: TSSB hastalarında çocukluk dönemi travmatik yaşam olaylarının yaygın olduğu ve çocukluk travmatik yaşantılarının TSSB semptom yoğunluğu üzerinde belirieyici olabileceği sonucuna varılmıştır.
