Surgical treatment of nongangreneous diabetic foot infections according to magnetic resonance imaging results

Abstract
More than 120 million people in the world have diabetes mellitus and too many of these subjects suffer from diabetic foot ulcers, which may eventually lead to an amputation. Given the high costs associated with a foot ulcer, this disorder is not only a major burden to the patient but also to the health care system. Diabetic foot is one of the important complications caused by peripheric vascular pathology, infection and neuropathy due to diabetes mellitus. Attention is needed for its diagnosis and treatment. The aim of this study is to evaluate the results of the treatment protocol based on the investigation of soft tissue and bone infection by Magnetic Resonance Imaging (MRI) and the resection/amputation of the infected bone in diabetic foot without gangrene. 95 diabetic foot without gangrene were evaluated in terms of soft tissue and bone infection by using MRI between November 1997-May 2000 in our clinic. Bone infection was diagnosed in 48 patients (Wagner Type-3). Below the knee amputation was performed in 5 of them according to the results of arteriography. Distal amputation including the infected bone was performed in 38 patients; osseous resection, open wound care and secondary closure were performed in 5. Below the knee amputation was necessary in 2 patients to whom Syme amputation was applied because of the posterior skin flap necrosis. No other problems that would require additional amputation or surgical operation were encountered in the remaining 41 patients during the twenty-six month follow-up period. The aimed treatment results can be reached without any necessity of long hospitalization period, repeated debridement operations, antibiotic usage and dressing follow-ups with the treatment protocol based on the evaluation of diabetic foot without gangrene by using MRI, discrimination of Wagner Type 3 foot, and resection of the infected bone.

Dünyada 120 milyondan fazla diyabet hastası bulunmaktadır ve çoğunda sonunda amputasyona kadar gidebilen diabetik ayak ülserleri gelişmektedir. Sebep olduğu yüksek tedavi giderleri nedeniyle yalnız kişi için bir sıkıntı olmayıp aynı zamanda sağlık sistemi içinde ağır bir yük oluşturmaktadır. Diabetus mellitusa bağlı nöropati, periferik vasküler patolojiler ve infeksiyonun birlikte oluşturduğu önemli komplikasyonlardan biriside diabetik ayaktır. Teşhis ve tedavisi özelliklidir. Bu çalışmada gangrensiz diabetik ayaklarda Magnetik Rezonans Görüntüleme (MRG) ile yumuşak doku ve kemik enfeksiyonu varlığının araştırılması ve infekte kemiğin rezeksiyonu/amputasyonu esasına dayanan tedavi protokolünün sonuçlarının değerlendirilmesi amaçlanmıştır. Kasım 1997-Mayıs 2000 yılları arasında 95 gangrensiz diabetik ayak, yumuşak doku ve kemik enfeksiyonu yönünden MRG ile değerlendirildi. 48 ayakta kemik enfeksiyonu (Wagner Tip-3) saptandı. Bu ayakların 5'ine arteriografi sonucuna göre dizaltı amputasyon uygulandı. Diğer 43 ayağın 38'üne enfekte kemiği içeren distal amputasyon, 5'ine osseöz rezeksiyon, açık yara bakımı ve sekonder kapatma uygulandı. Syme amputasyon uygulanan 2 olguda gelişen posterior cilt flebi nekrozu nedeniyle diz altı amputasyon gerekti. Ortalama 26 aylık takipte diğer 41 ayakta ek amputasyon ya da cerrahi girişim gerektirecek problem görülmedi. Gangrensiz diabetik ayakların MRG ile değerlendirilmesi, Wagner Tip-3 ayakların ayırt edilmesi ve infeksiyon saptanan kemiğin rezeksiyonu esasına dayanan tedavi protokolü ile gereksiz pansuman takipleri, antibiyotik uygulamaları, tekrarlanan debritman ameliyatları, uzun süreli Hospitalizasyon gibi yüksek maliyetli uygulamalara gerek kalmaksızın, hedeflenen tedavi sonucuna varılması mümkündür.
