Evaluation of patients with type 1 and type 2 diabetes mellitus regarding biopsychosocial factors

Abstract

Objective: In this study, comparison of patients with Type 1 and 2 Diabetes Mellitus(DM), regarding their frequency of living stressful lifes, levels of HbA<sub>1c</sub>, depression and psychiatric symptoms, was aimed. Method: Patients who consult to the Endocrinal Diseases Clinic of the GATA Haydarpaşa Medical Training Hospital in 1999 with a diagnosis of Type 1 and 2 DM were included in this study. Patients were given an interview questionnaire which examines the sociodemographic data, disease history and stressor life events as well as Beck Depression Inventory (BDI), Short Symptom Inventory (SSI) and their HbA1c investigation was carried. Results: There were significantly more young, single males in Type 1 group. In Type 2 group mean disease period, frequency of comorbidity and stressor life event frequency in the previous 3 months were significantly more. HbA<sub>1c</sub> levels, symptom seriousness index (ssi) and total symptom index (tsi) mean scores were significantly higher in Type 1 group. The patients who had higher scores than the entire groups arithmetical average of HbA<sub>1c</sub> scores, when compared to the lower HbA<sub>1c</sub> score group, had significantly higher frequency of reported stressor life event in the previous year and mean depression score. Total (SSI) and mean ssi scores were significantly higher in the group who reported stressor life event in the previous 3 months. Discussion: In this study with diabetic patients, it can be suggested that there is a close association between, stressor lifes and glucose metabolism; and diabetics with a diagnosis of Type 1 who report more stressor life events have a tendency to have more psychiatric symptom frequency.

Özet

Amaç: Bu çalışmada Tip 1 ve 2 diyabetes mellituslu (DM) hastaların stresli yaşantıları yaşama sıklığı, HbA 1c düzeyleri, depresyon ve psikiyatrik semptomlar açısından karşılaştırılması amaçlanmıştır. Gereç ve Yöntem: 1999 yılında GATA H. Paşa Eğitim Hastanesi Endokrinoloji ve Metabolizma Hastalıkları Servisine başvuran, DM Tip 1 ve 2 tanısı alan hastalar çalışmaya alınmıştır. Deneklere sosyodemografik özellikleri, hastalık öyküsünü ve stresör yaşam olaylarını sorgulayan bir görüşme anketi ile Beck depresyon envanteri (BDE), Kısa semptom envanteri (KSE) uygulanmış ve HbA 1c tetkiki yapılmıştır. Bulgular: Tip 1 grubunda genç, bekar ve erkek denekler anlamlı düzeyde daha sıktı. Tip 2 grubunda hastalık süresi ortalaması, komorbidite sıklığı ve son 3 ay içinde stresör yaşam olayı sıklığı anlamlı düzeyde daha fazla idi. HbA 1c düzeyi, rahatsızlık ciddiyeti indeksi (rci), belirti toplamı indeksi (bti) ortalama puanları Tip 1 grupta anlamlı düzeyde yüksek bulunmuştur. Tüm çalışma grubunun HbA 1c düzeyi aritmetik ortalamasının üstünde değere sahip olan hastalarda son 1 yıl içinde bildirilen stresli yaşam olayı sıklığı ve depresyon puan ortalaması; HbA 1c düşük gruba göre anlamlı düzeyde daha fazla bulunmuştur. Son 3 ay içinde stresli yaşam olayı tanımlayanlarda KSE toplam ve rci ortalama puanları anlamlı düzeyde daha yüksekti. Sonuç: Diyabetik hastalarla yapılan bu kesitsel çalışmada stresör yaşantılar ile glukoz metabolizmasının yakın bir ilişki gösterdiği; Tip 1 tanısı alan, yaşam olayı tanımlayan diyabetiklerin daha fazla psikiyatrik semptom sıklığına sahip oldukları söylenebilir.
