Changes in serum cholesterol and triglyceride levels in chronic hepatitis B patients treated with inferferon-alpha

Abstract
Interferons decrease lipoprotein lipase, hepatic triglyceride lipase and plasma cholesteryl ester protein activity. Decrease in lipoprotein lipase activity causes decrease in very low density lipoprotein clerance. Thus changes occur in serum cholesterol and tryglyceride levels. The aim of this study was to investigate if serum cholesterol and triglyceride levels were affected in patients with chronic hepatitis B treated with interferon-$\alpha$ ( IFN-$\alpha$). Fourty patients (35 male and 5 female) with chronic hepatitis B treated with 4,5/5 MU IFN-$\alpha$ daily were enrolled in our study. Serum cholesterol and tryglyceride levels of the patients were assayed before the therapy, in 1st, 3rd, and 6th months of the therapy and 6 months after cessation of therapy. The baseline serum cholesterol levels (186 ± 38.34 mg/dl) of the patients transiently decreased and the average of the three levels assessed during the therapy was 147 ±23.20 mg/dl (p<0.001). There was no significant difference between serum cholesterol levels before the therapy and 6 months after the therapy ended (p>0.05). The serum triglyceride levels before the therapy (120 ± 51.97 mg/dl) increased significantly with the therapy (226 ± 163.63 mg/dl, p<0.001) and decreased to their baseline levels (110 ± 60.56 mg/dl, p<0.001) 6 months after cessation of the therapy. In our study group, no patient suffered from severe hypertriglyceridemia. However, since severe hypertriglyceridemia was reported in previous studies with IFN-$\alpha$ therapy, the serum lipid profile in such patients should be monitorized to be aware of severe hypertriglyceridemia.

İnterferonlar lipoprotein lipaz, hepatik trigliserid lipaz ve kolesteril ester transfer protein aktivitesini azaltmaktadır. Lipoprotein lipaz aktivitesindeki azalma, çok düşük yoğunluklu lipoprotein klerensinde azalmaya neden olur. Böylece serum kolesterol ve trigliserid düzeylerinde değişiklikler meydana gelir. Bu çalışmanın amacı interferon-$\alpha$ (IFN-$\alpha$) ile tedavi edilen kronik hepatit B'li olgularda serum kolesterol ve trigliserid düzeylerinin etkilenip etkilenmediğini araştırmaktır. Günlük 4,5 / 5 MU IFN-$\alpha$, subkütan uygulanan 35'i erkek, 5 kadın 40 kronik hepatit B infeksiyonlu hasta çalışmaya dahil edildi. Tedaviye başlamadan önce ve tedavinin 1.,3. ve 6. ayları ile tedavinin bitiminden 6 ay sonra hastaların serum kolesterol ve trigliserid düzeyleri ölçüldü. Hastaların tedavi öncesi 186 ± 38.34 mg/dl olan serum kolesterol düzeyleri ortalaması tedavi süresince geçici olarak azaldı ve tedavi süresince ölçülen üç değerin ortalaması 147 ± 23.20 mg/dl olarak saptandı ( p 0.05). Tedavi öncesi serum trigliserid düzeylerinin (120 ± 51.97 mg/dl) tedavi ile belirgin şekilde yükseldiği (226 ± 163.63 mg/dl, p<0.001) ve tedavinin bitiminden altı ay sonra tedavi öncesi düzeylerine düştüğü (110 ± 60.56 mg/dl, p<0.001) gözlemlendi. Hiçbir hastada şiddetli hipertrigliseridemi saptanmadı. Ancak benzer çalışmalarda IFN tedavisi ile serum trigliserid düzeylerinde patolojik yükselmeler saptanması nedeniyle; IFN uygulanan hastalarda aşırı hipertrigliserideminin erken saptanabilmesi amacıyla, kan yağ profilinin izlenmesi gerekmektedir.
