Hypothalamo-pituitary-adrenal axis and catecholamine systems in posttraumatic stress disorder with comorbidity

Abstract

It has been assumed that there is a decrease in the activation of HPA system and an increase in the activation of catecholaminergic system in Posttravmatic Stres Disorder (PTSD), but this situation is uncertain if it has got a co-morbit psychiatric disorder. The aim of the present study was to evaluate the variations of HPA and catecholamine system in PTSD with comorbidity. Thirty-three male patients with chronic PTSD according to DSM III-R criteria were evaluated. Patients were assigned into the groups according to the presence of co-morbid disorders. Serum cortisol levels were determined before and after dexamethasone 1 mg, in addition, cortisol, adrenaline, noradrenaline and dopamine levels were assessed in 24 hours urine. Nine (28.1 %) of 32 subjects were nonsupressors. Nonsupression was found in 3 (27.3 %) of 11 subjects without any comorbid disorder and 1 (16.7 %) of 6 subjects with comorbid depression. The HPA and catecholamine system activities were not significantly different among groups, but except for the elevation of 24 hours urine cortisol/adrenaline in those with comorbid depression. We suggest that having a comorbid psychiatric disorder with PTSD not cause any change in the activation of HPA and catecholamine system.

Özet

Travma Sonrası Stres Bozukluğu (TSSB)nda aktivasyonun, Hipotalamo-Pitiüter-Adrenal (HPA) sistemde düşük, katekolaminerjik sistemde ise yüksek olduğu ileri sürülmektedir. Komorbidite varlığında ise durum tartışmalıdır. Bu çalışmada TSSB'na komorbidite eklendiğinde, HPA ve katekolaminerjik sistemde meydana gelen değişiklikler araştırılmıştır. DSM III-R kriterlerine göre kronik TSSB tanısı konan 33 erkek hasta çalışmaya alınmıştır. Olgular, komorbid bozukluklar yönünden gruplara ayrılmıştır. Bir mg'lık deksametazon öncesi ve sonrası serum kortizolleri; 24 saatlik idrarda kortizol, adrenalin, noradrenalin ve dopamin düzeyleri çalışılmıştır. Komorbid bozukluğu olmayan 11 olgunun 3'ünde (%27.3), depresyon komorbiditesi olan 6 olgunun 1'inde (%16.7) olmak üzere, veri toplanabilen toplam 32 olgunun 9'unda (%28.1) deksametazon sonrası supresyon oluşmamıştır. Depresyon komorbiditesi olanlarda 24 saatlik idrarda kortizol/adrenalin oranı yüksek bulunmuştur. Bunun dışında, gruplar arasında HPA aks ve katekolaminerjik sistemde fark bulunmamıştır. TSSB'na komorbidite eklendiğinde HPA ve katekolaminerjik sistemde değişiklik olmamaktadır
