Risk assessment of BRCA 1 and BRCA 2 gene mutations in hereditary breast cancer cases

Abstract
Breast cancer is the most common malignancy affecting women. 5-10% of breast cancers are hereditary. Hereditary forms are frequently occurs with the hereditary mutations of BRCA 1 and BRCA 2 genes. To date, hundreds of germline mutations have been identified in these genes. The genetic mechanism of hereditary breast cancer is not fully understood. The techniques applied in the analysis of these genes is very time consuming. Detecting the all mutations is impossible because of the technique applied in these cases. The cost of these techniques are still high and all the hereditary mutations do not always develop cancer. Consequently, the proband may have some psychological problems. Therefore probability models in the analyses of BRCA 1 and 2 genes are used for the evaluation of breast cancer. BRCA 1 and BRCA 2 findings are used for the prediction of risks. Here the risk assessment of BRCA I and BRCA II genes were analyzed in 21 family, referred to our department with hereditary breast cancer diagnosis, with respect to the data of Cough at all, Shattuck-Eidens at al, and Vahteristo at al. In 17 families, the assessment of hereditary mutation of BRCA 1 and BRCA 2 genes was observed as high as approximately %2, normally observed in non-familial breast cancer cases. One family diagnosed as Li-Fraumeni Syndrome with pedigree analysis and hereditay p53 mutation results. Two cases with breast cancer diagnosis below the age of 40, and one case with breast cancer diagnosis over the age of 40, the risk of BRCA 1 gene mutation was found as 6.7% and 17.4% in one family. The risk of BRCA 1 and BRCA 2 mutations were observed as 30% in this family. One case with breast cancer diagnosis under the age of 40, two cases with breast cancer diagnosis over the age of 40, the risk of BRCA 1 gene mutation were found as 6.7% and 17.4%. The risk of BRCA 1 and BRCA mutations was observed lower than 10% in this family. In another family two cases had breast cancer diagnosis over the age of 40, two cases had breast cancer diagnosis below the age of 40, one case had ovarian cancer diagnosis over the age of 40 and one case had ovarian cancer diagnosis below the age of 40. One case died at age of 10 because of a tumor in kidney and one case died at age of 16 because of "nasopharinx" carcinoma. The risk assessment of a BRCA 1 gene mutation was observed as 71.4% and 55%. The risk assessment of BRCA 1 and BRCA 2 mutations was observed as 50% in this family. In 80% cases, finding the lowest risk value (%2) shows us the importance of the assessment of mutation risk of BRCA 1 and BRCA 2 genes. We suggest to use these results in genetic counseling of these families.

Meme kanseri, kadınlarda gözlenen kanserler içinde ilk sıradadır ve bu kanserlerin yaklaşık %5-10'u kalıtsaldır. Herediter meme kanseri olgularında BRCA 1 ve BRCA 2 mutasyonları sıktır. Bu iki gene ait yüzlerce farklı mutasyonun tanımlanması, meme kanserine ait etyolojinin tümü ile ortaya konamaması, bu analizlerin günümüz teknikleri ile yapılmasındaki güçlük, tetkiklerin yüksek maliyeti, elde edilen mutasyonun kanser nedeni olup olmama yönündeki belirsizlikler, varolan bir mutasyonu bulamama olasılığı, tüm bu nedenlere bağlı olarak testlerin uygulandığı ailelerde olabilecek psikolojik sıkıntılar nedeni ile ailesel meme kanseri düşünülen olgularda herediter BRCA 1 ve BRCA 2 mutasyonlarının araştırılmasında belli kriterlere göre risk hesaplanmaları önem kazanmıştır. Burada herediter meme kanseri ön tanısı ile refere edilen 21 aile Cough ve arkadaşları, Shattuck-Eidens ve arkadaşları, Vahteristo ve arkadaşlarının bulguları ışığında risk analizine alınmış ve 17 ailede (%80) bu genlere ait herediter mutasyon bulunma olasılığının ailesel olmayan meme kanserii (non-hereditary breast cancer) olgulardaki risk kadar (yaklaşık % 2) olduğu tespit edilmiştir. Bir aile pedigree analizi sonuçları ve taşıdığı herediter p53 mutasyonu nedeni ile Li-Fraumeni Sendromu tanısı almıştır. 40 yaş altı iki ve 40 yaş üstü bir meme kanseri tanısı almış bir ailede BRCA l mutasyonu bulunma olasılığı %6.7 ve %17.4 olarak BRCA l ve BRCA II mutasyonu bulunma olasılığı %30 olarak değerlendirilmiştir. 40 yaş altı bir ve 40 yaş üstü iki meme kanseri tanısı almış diğer bir ailede BRCA 1 mutasyonu bulunma olasılığı %6.7 ve %17.4 olarak değerlendirilmiş, BRCA 1 ve BRCA 2 genlerine ait mutasyon bulunma olasılığı %10 un altında bulunmuştur, iki 40 yaş altı, iki 40 yaş üstü meme kanseri tanısı almış, bir 40 yaş altı ve bir 40 yaş üstü over kanseri tanısı almış, bir kişinin 10 yaşında böbrek tümörü, bir kişinin 16 yaşında "nasopharinx" kanseri nedeni ile kaybedildiği bir ailede BRCA 1 mutasyonu bulunma olasılığı %71.4 ve % 55, BRCA 1 ve BRCA 2 mutasyonu bulunma olasılığı ise % 50 olarak bulunmuştur. Ailesel meme kanseri ön tanısı ile refere edilen olguların yaklaşık %80 inde en düşük risk oranının gözlenmesi, diğer olgularda ise risk yüzdelerinin bulunması olgularda herediter BRCA 1 ve BRCA 2 genlerine ait risk hesaplanmasının ve o bulgulara göre genetik danışma verilmesinin önemini göstermektedir.
