Factors and causes accepted as potential errors in medication order

Abstract
To give and get the medication orders is one of the most important stages in the medication application process. Determination of the cases and reasons which may cause potential errors on this stage will be basis for the measurements which will be taken. This study which was planned as sectionally included 56 nurses working at the clinics of internal diseases and surgery. Their opinions for the research topic were asked. The data were obtained through a form consisted of give parts and they were evaluated in percentages. Consequently, it has been found that the nurses think that the mistakes might be made in the orders which are given as written at the very least and they might occur mostly while recording orally given orders. The nurses have started that the stuations such as the verbal orders which are not written and fatigue are the most important factors that can cause mistakes. They have also emphasized that the priority is for the written procedures of giving and getting orders.
Özet
ilaç istemlerinin verilmesi ve alınması ilaç uygulama sürecinin önemli bir basamağını oluşturur. Bu basamakta potansiyel hatalara neden olabilecek durumların ve nedenlerinin saptanması alınacak önlemlere esas oluşturacaktır. Kesitsel olarak planlanan bu çalışma Gülhane Askeri Tıp Akademisi Eğitim Hastanesinin iç ve cerrahi kliniklerinde çalışan 56 hemşireyi kapsamıştır. Çalışmada hemşirelerin ilaç istemlerinin alınmasında potansiyel hata olarak gördükleri durumlar ve nedenlerine yönelik görüşler alınmıştır. Veriler beş bölümden oluşan veri toplama formu ile elde edilmiş olup, yüzdelikleri alınarak değerlendirilmiştir. Sonuç olarak hemşirelerin, en az potansiyel hataya neden olabilecek durumların yazılı verilen istemlerde, en fazla potansiyel hataya neden olabilecek durumların ise istemin hemşire kayıtlarına geçirilişinde yapılabileceği görüşünde oldukları saptanmıştır. Hemşireler, sözel verilen istimlerin yazılı hale getirilmemesi ve yorgunluk gibi durumların hatalara neden olabilecek en önemli faktörler olduğunu belirtmişlerdir. Alınabilecek öncelikli önlemin ise, istemin verilmesi ve alınmasına yönelik yazılı prosedürlerin oluşturulması olduğunu vurgulamışlardır.
