Modular megaprosthesis for primary malign and metastatic proximal femoral tumor resection: Short-term results

Abstract

Nine patients with the diagnosis of malign bone tumor of the proximal femur were treated with the wide surgical resection of tumor and reconstruction with modular megaprosthesis. Six of the patients were male and 3 were female with an average age of 64.1 years old. The mean length of proximal femoral resection was 13.1 cm. Modular megaprosthesis was applied with a cemented femoral stem and bipolar acetabulercomponent. Gluteus médius was attached to the prosthesis which surrounds soft tissues in all patients. At the end of the follow-up period, clinical evaluation of the patients was made according to the Musculoskeletal Tumor Society (MSTS) scoring system. One patient who died of causes related to the tumor was excluded from this study. The average follow-up period of the rest 8 patients was 19.8 months There wasn't any local reoccurence. The mean MSTS score was 23.75 (%79.2) at the end of the follow-up period. In the short-term period after surgery, one patient had recurrent dislocations and revision surgery was applied.

Özet

Proksimal femurda malign kemik tümörü tanısı konulan 9 hastada tümörün geniş cerrahi rezeksiyonu ve modüler megaprotez ile rekonstrüksiyon ameliyatı yapıldı. Hastalardan 6'sı erkek, 3'ü kadın ve ortalama yaş 64.1 idi. Hastalarda uygulanan proksimal femoral rezeksiyonun ortalama uzunluğu 13.1 cm oldu. Modüler megaprotez tüm hastalarda çimentolu femoral stem ve bipolar asetabuler komponent ile uygulandı. Tüm olgularda gluteus mediusun protez ve çevre yumuşak dokulara rekonstrüksiyonu sağlandı. Hastaların takip süresi sonunda klinik değerlendirmeleri Musculoskeletal Tumor Society (MSTS) skorlaması kullanılarak yapıldı. Ameliyat sonrası 2. ayda tümöre bağlı nedenlerden ölen 1 hasta çalışma dışında bırakıldı. Diğer 8 hastanın ortalama takip süresi 19.8 ay oldu. Hiçbir olguda lokal rekürrens izlenmedi. Takip süresi sonunda hastalara ait ortalama MSTS skor puanı 23.75 (%79.2) bulundu. Bir olguda ameliyat sonrası erken dönemde tekrarlayan disloksyon gelişti ve revizyon cerrahisi uygulandı.
