The prevalence of hepatitis B and hepatitis C in the patients who received orthopaedic surgery: 10 years retrospective study

Abstract

Objective: In this study it was aimed to determine the incidence and the risk factors of hepatitis B (HBV) and hepatitis C (HCV) viruses infections by searching the data of our clinic for ten years. Methods: In our clinic, elective or emerge surgical procedure performed cases were evaluated retrospectively between March 1996 and May 2006. In those cases HBV, HCV and HIV carriers and the existence of the risk factors were searched. Results: The records of total 10,712 patients were evaluated. In these cases 395 of those positive HBsAg, 208 had positive anti HCV, and none of them had positive HIV. The most common risk factor was the story of the surgical interference (included dental treatment). Statistically, positive HBsAg was found significantly higher than positive anti HCV. Conclusion: In our study, the prevalence of HBV was nearly the same of mean value of our country, but the prevalence of HCV was found higher. This situation requires more care by taking serious preventions to the HCV, especially in cases whose virus serology is undefined and emerge surgical procedure performed.

Özet

Amaç: Bu çalışma ile kliniğimizin 10 yıllık verileri incelenerek hepatit B (HBV) ve hepatit C (HCV) virüs enfeksiyonlarının insidans ve risk faktörlerini değerlendirmek amaçlanmıştır. Yöntem: Mart 1996-Mayıs 2006 tarihleri arasında kliniğimizde elektif veya acil cerrahi girişim uygulanan olguların kayıtları retrospektif olarak değerlendirildi. Olgularda HBV, HCV ve HIV taşıyıcılığı ve risk faktörlerinin varlığı araştırıldı. Bulgular: Toplam 10,712 hastanın kayıtları retrospektif olarak değerlendirildi. Bu olguların 395’inde HBsAg (+), 208’inde Anti-HCV (+) olarak saptanırken hiç birinde HIV (+)’liği belirlenmedi. En sık görülen risk faktörü, cerrahi girişim (diş tedavisi dâhil) öyküsüydü. Hastalarda istatistiksel olarak HBsAg pozitifliği Anti HCV pozitifliğinden anlamlı olarak yüksek saptandı. Cinsiyetler arasında HBsAg pozitifliği ve Anti HCV pozitifliği arasında anlamlı bir fark saptanmadı. Sonuç: Çalışmamızda HBV prevelansı ülkemizdeki ortalama değer ile uyumlu iken HCV prevelansı daha yüksek saptanmıştır. Bu durum önemli bir sağlık problemi olan HCV’ye karşı daha dikkatli olmayı ve özellikle acil girişim uygulanan, virus serolojisi belirlenmemiş olgularda daha ciddi önlemler alınmasını gerektirmektedir.
