The Investigation of Chlamydia trachomatis and Ureaplasma urealyticum Coinfections in HPV Positive and HPV Negative Women

Abstract

Introduction: Human papillomavirus (HPV) is the primary cause of cervical cancer. Chlamydia trachomatis together with HPV is a potential cofactor for the development of cervical intraepithelial neoplasia. Ureaplasma urealyticum, which is found commensally in the urogenital region, is considered to contribute to urogenital infections. The aim of this study was to investigate C. trachomatis and U. urealyticum coinfections in HPV positive and HPV negative women. Materials and Methods: Cervical swab samples were collected from 300 females aged 20-65 years. Real-time PCR (Anatolia Geneworks, Turkey) test was used for HPV genotyping. C. trachomatis and U. urealyticum infections were detected by in-house PCR in HPV positive (n= 200) and HPV negative (n= 100) women. Results: HPV 16 (53%) was the most dominant HPV genotype in 200 HPV positive women, followed by HPV 18 (30%), HPV 68 (10.5%), HPV 56 (10%), HPV 45 (9%), HPV 52 (6%), HPV 39 and HPV 51 (5%), HPV 66 (3.5%), HPV 35 and HPV 58 (2.5%), and HPV 33 (1.5%). C. trachomatis and U. urealyticum were detected to be 22 (11%) and 61 (30.5%), respectively in HPV positive women, while it was found to be 8 (8%) and 23 (23%), respectively in HPV negative women. C. trachomatis and U. urealyticum coinfection was found in 1.5% (3/200) of HPV positive women and 4% (4/100) of HPV negative women. Conclusion: C. trachomatis and U. urealyticum coinfection was found at a relatively high rate in high-risk HPV positive female patients. In order to understand the role of coinfections in cervical cancer pathogenesis, further studies including a control group may be instructive.

Giriş: Human papillomavirüs (HPV) servikal kanserin primer sebebidir. HPV ile birlikte Chlamydia trachomatis servikal intraepitelyal neoplazi gelişimi için potansiyel bir kofaktör olarak rol oynamaktadır. Ürogenital bölgede kommensal olarak bulunan Ureaplasma urealyticum’un ise ürogenital infeksiyonlara katkıda bulunduğu kabul edilmiştir. Bu çalışmanın amacı HPV pozitif ve HPV negatif kadınlarda C. trachomatis ve U. urealyticum koinfeksiyonlarının araştırılmasıdır. Materyal ve Metod: Servikal sürüntü örnekleri 20-65 yaş arasındaki 300 kadından toplandı. HPV genotiplemesi için gerçek zamanlı polimeraz zincir reaksiyonu (rt-PCR) (Anatolia Geneworks, Turkey) testi kullanıldı. HPV pozitif (n= 200) ve HPV negatif (n= 100) kadınlarda C. trachomatis ve U. urealyticum infeksiyonları in-house PCR testi ile tespit edildi. Bulgular: HPV pozitif 200 kadında en baskın HPV genotipi HPV 16 (%53) olup, bunu HPV 18 (%30), HPV 68 (%10.5), HPV 56 (%10), HPV 45 (%9), HPV 52 (%6), HPV 39 (%5), HPV 51 (%5), HPV 66 (%3.5), HPV 35 (%2.5), HPV 58 (%2.5) ve HPV 33 (%1.5) izlemiştir. C. trachomatis ve U. urealyticum HPV pozitif kadınlarda sırasıyla %11 (n= 22) ve %30.5 (n= 61) oranında, HPV negatif kadınlarda ise sırasıyla %8 (n= 8) ve %23 (n= 23) oranında bulunmuştur. C. trachomatis ve U. urealyticum koinfeksiyonu HPV pozitif kadınlarda %1.5 (3/200) oranındayken, HPV negatif kadınlarda %4 (4/100) oranında görülmüştür. Sonuç: C. trachomatis ve U. urealyticum koinfeksiyonunun yüksek risk HPV pozitif kadınlarda oldukça yüksek oranda bulunmuştur. Servikal kanser patogenezinde koinfeksiyonların rolünün anlaşılması için bu konuda kontrol grubunu da içeren daha ileri araştırmalar yol gösterici olabilir.
