A case of diffuse tinea incognito associated with long-term systemic steroid use

Abstract

A 26-year-old male patient presented with a history of diffuse pruritus and rash on the body. Dermatological examination showed diffuse erythematous plaques with squamous surfaces on his hands, feet, inguinal area and intergluteal area. Subungual hyperkeratosis, onychodystrophy and onycho-dyschromia were observed on all hand and foot nails. The patient had been using intra-muscular betamethasone dipropionate weekly for 8 years. Native examination of the lesions on the hands, feet, inguinal area, hand and foot nails were evaluated positive, and Trichophyton rubrum grew in the fungal culture of the soles. The patient was diagnosed as diffuse tinea incognito associated with long-term systemic steroid use, and systemic and double topical antifungal treatment was started.

Özet

26 yasında erkek hasta vücudunda yaygın kasıntı ve döküntü öyküsü ile polikliniğimize basvurdu. Dermatolojik muayenede hastanın ellerinde, ayaklarında, inguinal bölge ve intergluteal bölgede yaygın eritemli üzeri skuamlı plaklar ve tüm el ve ayak tırnaklarında subungal hiperkeratoz, onikodistrofi ve onikodiskromi mevcuttu. Hastanın anamnezinden sekiz yıldan beri her hafta intramuskular betametazon dipropionate kullandığı öğrenildi. Lezyonlardan yapılan nativ inceleme sonucu el, ayak, inguinal bölge, el ve ayak tırnakları pozitif olarak değerlendirilirken yapılan mantar kültürü sonucunda ayak tabanında Trichophyton rubrum üredi. Hastaya uzun süreli sistemik steroid kullanımına bağlı gelisen yaygın bir tinea inkognito tanısı konularak, sistemik ve ikili topikal antifungal tedavisi baslanıldı.
