Hearing screening test results of newborns at high risk for developmental delay

Abstract
Objective: The newborns hospitalized in neonatal intensive care units are presumptively at a higher risk for developmental delay. In our study we aimed to evaluate the frequency of hearing loss in neonates with high risk. Methods: In our study 7067 noenates who were followed-up in the neonatology clinic between January 2005 to December 2007 were included in this study. The otoacoustic emission and auditory brain stem potential (BERA) tests were performed to evaluate and screen the hearing loss in infants. Results: Totally 277 neonates (4%) were detected to have a hearing loss and referred to a higher level hospital for further investigations. During the period of 2005 to 2006, confirmed hearing loss rate was found to be 0.91% and 0.96% in 2007, respectively. The most prominent risk factor for hearing loss was exchange tranfussion requiring hyperbilirubinemia (45.8%). Conclusion: Hearing loss was found to be common in high risk newborns, so screening should be performed in all patients to detect the cases with hearing loss.

Amaç: Yenidoğan yoğun bakımına yatan hastalar gelişimsel açıdan riskli bebeklerdir. Çalışmamızda gelişimsel açıdan riskli yenidoğanlarda işitme kaybı oranının belirlenmesi amaçlandı. Yöntemler: Yenidoğan yoğun bakıma Ocak 2005 ile Aralık 2007 tarihleri arasında yatırılarak izlenen toplam 7.067 bebekte otoakustik emisyon ve işitsel uyarılmış beyin sapı potansiyeli (BERA) ile işitme taraması yapıldı. Bulgular: İki yüz yetmiş yedi bebeğin (%4) taramadan kaldığı ve ileri inceleme için sevk edildiği saptandı. 2005-2006 yıllarında kesinleşen işitme kaybı oranı %0.91 2007 yılında ise %0.96 olarak belirlendi. İşitme kayıplarında ilk sıradaki risk faktörü exchange gerektirebilecek düzeyde hiperbilurubinemi varlığı olarak saptandı (%45.8). Sonuç: İşitme kaybının riskli yenidoğanlarımızda yüksek oranda saptanması bu yaş grubunda işitme taramasının önemini ortaya koydu.
