Abstract
Alzheimer's disease (AD) is a neurodegenerative disorder of unknown etiology characterized by progressive mental deterioration and loss of various cognitive functions. Several risk factors are encountered in the etiology, and recently studies about genetic inheritance have gained importance. Although exposure to anesthesia has also been blamed, a direct relationship has not yet been found. It has been proposed that a decrease in cholinergic innervation plays a role in the pathogenesis of AD, along with the accumulation of certain peptides and proteins especially in cerebral cortex and limbic system, which for various reasons results from oligomerisation. In AD, this peptid is b-amiloid. The diagnosis is generally based on clinical observation, whereas treatment is symptomatic and aims to support cholinergic innervation as well as improve the prognosis. Although no specific complications of anesthesia have been reported for AD, it is known that general anesthesia depresses the cholinergic system and some volatile anesthetics cause the oligomerisation of peptides as well. Barbiturates and opioids depress the cholinergic system, whereas propofol and remifentanil have no effect in clinical doses. Regional anesthesia has not been found beneficial over general anesthesia. As the patients are generally old, the principles of geriatric anesthesia must be encountered. Centrally acting anticholinergics must be avoided. Studies searching for an ideal anesthetic approach continue taking the cholinergic system as a baseline.

Alzheimer hastalığı (AH) nedeni aydınlatılamamış bir nörodejeneratif hastalık olup özellikle ilerleyici bellek bozukluğu ve çeşitli kognitif işlev kaybı ile karakterizedir. Etiyolojisinde çeşitli risk faktörleri bildirilmiştir, günümüzde genetik geçişle ilgili çalışmalar önem kazanmıştır. Sık anestezi uygulamaları suçlanmışsa da direkt ilişki bulunamamıştır. Alzheimer hastalığının patogenezinde kolinerjik iletideki azalmanın etkili olduğu, bazı protein ve peptidlerin çeşitli nedenlerle oligomerizasyonu sonucu özellikle serebral korteks ve limbik sistemde birikmelerinin ileti kaybında rol oynadığı bildirilmiştir. Alzheimer hastalığında bu madde b-amiloid peptiddir. Tanı genelde klinik bulgulara dayanır, tedavisi ise semptomatik olup serebral kolinerjik iletiyi artırmayı ve hastalığın ilerlemesini engellemeyi amaçlar. Alzheimer hastalığına özgün bir anestezi komplikasyonu bildirilmemekle birlikte genel anestezinin kolinerjik sistemi baskıladığı, bazı volatil anestetiklerin peptid oligomerizasyonuna yol açtığı bilinmektedir. Barbituratlar ve opioidler kolinerjik sistemi baskılarken klinik dozlarda propofol ve remifentalinin bu sistemle etkileşimi bildirilmemiştir. Genel ve bölgesel anestezinin birbirine üstünlüğü gösterilmemiştir. Hastalar genellikle yaşlı olduğu için geriatrik anestezi prensiplerine uyulmalı, etkileşim açısından kullanılan ilaçlar öğrenilmelidir. Santral etkili antikolinerjiklerden kaçınılmalıdır. Alzheimer hastalığında ideal anestezi yöntemi ve anestetik arayışı kolinerjik sistem temel alınarak devam etmektedir.
