The importance of intervertebral disc rupture in cervical facet dislocatons

Abstract
Aim: Facet dislocations of the cervical spine are frequent and and serious injuries, which may lead to quadriplegia and death. The aim of this study is to determine the ratio of the facet dislocations and the coincidence of the traumatic intervertebral disc ruptures among the subaxial cervical spine injuries. Besides, the impact of the traumatic disc rupture on the surgical treatment of these injuries is also discussed. Patients and Methods: Thirty-nine patients with subaxial cervical spine injuries who treated in our clinic in the last ten years have been reviewed retrospectively. Unilateral and bilateral facet dislocations were found in 23 (59%) patients. Mechanisms of injury, injured levels, and neurological status of the patients, the presence of traumatic disc ruptures and the preferred treatment methods were evaluated. Results: Traumatic ruptures of the intervertebral discs were found in 80% of the patients with facet dislocations in our study. Thirteen per cent of the patients in our study were treated non-operatively while 22% of them were treated with anterior disc removal and anterior fusion, 30% of them were treated with anterior disc removal and anterior-posterior fusion and 35% of them were treated with closed reduction and posterior fusion. Conclusions: Goal of the treatment is to reduce and to stabilize the spinal column in facet dislocations. Neurological status of the patient should be prevented during the treatment. Determination of any disc rupture at the initial period may alter the treatment option in these injuries. Neurological complications may be prevented with the excision of the intervertebral disc before any reduction attempt in the patients with incomplete neurological deficits and in neurologically intact patients.

Amaç: Servikal omurgada faset çıkıkları sık görülen, tanı ve tedavisinde güçlüklerle karşılaşılan, kuadripleji ve ölümle sonuçlanabilen ciddi yaralanmalardır. Bu çalışmada amaç subaksiyel servikal omurga yaralanmaları içinde faset çıkıklarının ve eşlik eden travmatik intervertebral disk rüptürlerinin oranını belirlemek, disk rüptürü ve faset çıkığı birlikteliğinin klinik duruma ve tedaviye olan yansımalarını tartışmaktır. Hastalar ve Yöntem: Kliniğimizde son 10 yılda subaksiyel servikal omurga yaralanması nedeniyle tedavi edilen 39 olgu retrospektif olarak incelendi. Olguların 23ʼünde (% 59) tek ve çift taraflı faset çıkığı saptandı. Yapılan incelemede faset çıkığı saptanan olguların yaralanma mekanizmaları, lezyon seviyeleri, nörolojik durumları, faset çıkıklarına travmatik intervertebral disk rüptürünün eşlik edip etmediği ve tercih edilen tedavi yöntemleri değerlendirildi. Sonuçlar: Yapılan inceleme sonucunda faset çıkığı olan olguların %80ʼinde travmatik intervertebral disk rüptürü olduğu saptandı. Serimizdeki olguların %13ʼü cerrahi dışı yöntemlerle tedavi edilirken, %22ʼsine anterior diskektomi ve anterior füzyon, %30ʼuna anterior diskektomi ve anterior-posterior füzyon, %35ʼine ise kapalı redüksiyon ve posterior füzyon uygulandığı görüldü. Çıkarımlar: Faset çıkığı olgularında tedavinin amacı redüksiyon ve stabilizasyonun sağlanmasıdır. Ancak bunu sağlarken mevcut nörolojik tabloyu daha da kötüleştirecek işlemlerden kaçınmak gerekir. Faset çıkığı olgularında intervertebral disk rüptürünün önceden saptanması tedavi protokolünü etkiler. Nörolojik defisiti olmayan ya da inkomplet nörolojik defisitli olgularda açık veya kapalı redüksiyon öncesi yaralanmış olan diskin çıkarılması olası nörolojik komplikasyonların önüne geçecektir.
