Comparison of the safety of ıntracameral cefuroxime and moxifloxacin ınjection for prophylaxis of endophthalmitis in phacoemulsification surgery

Abstract

Purpose: The aim of this study was to evaluate and to compare the safety of intracameral cefuroxime and moxifloxacin injection at the end of cataract surgery for prophylaxis of endophthalmitis. Materials and Methods: This prospective study is comprised of 60 eyes of 30 patients who had undergone bilateral but different day phacoemulsification surgery and another 30 patients who had unilateral phacoemulsification. The patients having bilateral surgery were randomly assigned to receive intracameral 1 mg/0.1 ml cefuroxime in one eye (Group 1), while in other eye, 250 µg/0.1 ml moxifloxacin was injected into the capsular bag at the end of surgery (Group 2). Patients who had unilateral phacoemulsification received intracameral BSS at the completion of the surgery (Group 3). Safety parameters, including visual acuity, intraocular pressure (IOP), corneal pachymetry, corneal clarity or edema, anterior chamber cells and flare and central foveal thickness measured by optical coherence tomography (OCT) were evaluated preoperatively and on the 1st , 7th, 30th and 90th days postoperatively. Corneal endothelial cell counts were assessed preoperatively and on the 30th and 90th days postoperatively. Results: There were no statistically significant difference between the groups in age, cataract hardness, phaco time, total operation time, fluid used( p>0.05). There were no significant difference between the groups in visual acuity, IOP, corneal edema, anterior chamber reaction, OCT results and endothelial cell counts. Conclusion: There was no anterior and posterior segment toxicity risk with intracameral injection of 1 mg/0.1 ml cefu roxime and 250 µg/0.1 ml moxifloxacin for prophylaxis of endophthalmitis after cataract surgery, and their intracameral injection were found to be as safe as BSS.

Amaç: Katarakt ameliyatlarında endoftalmi profilaksisi için ameliyat sonunda intrakamaral sefuroksim ve moksifloksasin kullanımının güvenilirliğini değerlendirmek ve karşılaştırmak amaçlandı. Gereç ve Yöntem: Bu geriye dönük çalışmaya bilateral kataraktı bulunan ve her iki gözüne farklı günlerde fakoemülsi fikasyon cerrahisi uygulanan 30 hastanın 60 gözü ve tek taraflı fakoemülsifikasyon uygulanan 30 hastanın 30 gözü alındı. Bilateral cerrahi uygulanan hastaların bir gözüne ameliyat sonunda intrakamaral 1 mg/0.1 ml sefuroksim verilirken (Grup 1), diğer gözlerine 250 µgr/0.1 ml moksifloksasin (Grup 2) verildi. Tek taraflı cerrahi uygulanan hastalara (Grup 3) ise ameliyat sonunda intrakamaral BSS verildi. Güvenilirlik parametreleri olarak, görme keskinliği, GİB, korneal pakimetri, korneal ödem, ön kamarada hücre ve flare ve optik koherens tomografi (OKT) ile saptanan merkezi foveal kalınlık ameliyat öncesi ile ameliyat sonrası 1., 7. ile 30. ve 90. günlerde değerlendirildi. Kornea endotel sayısı ameliyat öncesi ile ameliyat sonrası 30. ve 90. günlerde değerlendirildi. Bulgular: Gruplar arasında yaş, katarakt sertlikleri, fako zamanı, tüm ameliyat süresi, kullanılan sıvı miktarı bakımın dan anlamlı fark yoktu (p>0.05). Üç grup arasında görme keskinliği, GİB, korneal ödem, ön kamara reaksiyonu, OKT so nuçları ve endotel hücre sayıları açısından istatistiksel olarak anlamlı bir fark yoktu. Sonuç: Katarakt cerrahisinde endoftalmi profilaksisinde intrakamaral 1mg/0.1ml sefuroksim ve 250 µgr/0.1 ml moksif loksasin ile ön ve arka segment toksisite riski olmadığı görülmüştür ve intrakamaral kullanımları BSS kadar güvenilir bulunmuştur.
