Determination of entropy values towards bispectral index values in the duration of recovery

Abstract
Aim: Our study was performed with the aim of determining state (SE) and response (RE) entropy values corresponding to bispectral index (BIS) values in patients undergoing general anesthesia. Patients and methods: Forty ASA I-II laparoscopic cholesistectomy cases were included in this study. The patients were divided randomly into two groups: group 1 BIS (n=20) and group 2 entropy RE/SE (n=20). In Group 1 BIS and in Group 2 RE/SE electrodes were applied to the forehead of the patients in the operating room. B‹S and RE/SE values were recorded at the end of anesthesia, at spontaneous eye opening, at the beginning of spontaneous respiration, at extubation, at the time the patient was conscious, at discharge from the postanesthesia care unit and when Aldrete≥9. Results: The entropy values at spontaneous respiration, at spontaneous eye opening, at extubation, at the time the patient was conscious, at discharge from the postanesthesia care unit and when Aldrete≥9 were found to be lower significantly than for the BIS values (p<0.05). Conclusion: In the recovery period, the monitoring of entropy values can be used as safely as the monitoring of BIS values.

Amaç: Çalışmamız genel anestezi alan olgularda bispektral indeks (BİS) değerlerine karşılık gelen durağan (state/SE) ve yanıt (response/RE) entropi değerlerini belirlemek amacıyla yapıldı. Yöntem: Laparoskopik kolesistektomi geçirecek ASA I-II grubunda 40 olgu çalışmaya dahil edilerek çalışma grupları grup 1 BİS (n=20), grup 2 entropi (RE/SE) (n=20) olarak rastgele seçimle belirlendi. Olgular operasyon odasına alındığında olguların alın bölgesine birinci grupta BİS (grup 1), ikinci grupta state-durağan entropi (SE)/response-yanıt entropi (RE) elektrodu (grup 2) uygulandı. Anestezinin sonlanmasını takiben her iki grupta da spontan göz açma, spontan solunumun geri döndüğü, ekstübasyondan hemen önceki, ekstübasyondan sonraki dönemlerdeki BİS ve entropi değerleri ile her iki grubun bilinç süreleri, derlenme ünitesi çıkış süreleri ve Aldrete derlenme skorlarının ≥9 olduğu süreler tespit edildi. Bulgular: Çalışma sonunda spontan solunum, spontan göz açma, ekstübasyon öncesi, ekstübasyon sonrası, bilincin açık ve Aldrete >9 olduğu dönemlerdeki derlenme skorlarının entropi değerlerinin BİS değerlerinden anlamlı şekilde düşük olduğu tespit edildi (p<0.05). Sonuç: Entropi değerlerinin BİS değerlerinden anlamlı olarak daha düşük olduğunu tespit ettiğimiz bu çalışma sonunda, derlenme döneminde BİS monitörizasyonu gibi entropi monitörizasyonunun da güvenle kullanılabileceğini düşünüyoruz.
