Association between giardiasis and atopy in children o low income families

Abstract
Objective: Giardiasis is a worldwide public health problem associated with significant morbidity. Giardia infection may cause food allergy by enhancing absorbotion of food allergens because it is corraleted to an increase of gastrointestinal permeability. In literature, the relationship with giardiasis and atopy is not clear. This study’s aim is to assess the relationship between giardiasis and atopy in children by conducting skin prick tests, and determining total and specific serum IgE levels. Materials and Methods: Stool samples from each child were examined by direct fluorescencantigen assay (DFA). Socio-economic status, hygiene conditions (the family’s income level, settle- ment, and the source of drinking water) and allergic history of all children were recorded using standardized questionnaires. Skin prick test with cow’s milk, egg and wheat floor antigens and common aeroallergens were performed. Total and specific serum IgE for cow’s milk, egg, wheat floor and mites, molds, tree, grass and weed pollens were measured. Results: 31 of 43 children who have gastrointestinal complaints were DFA positive for Giardia lamblia. The total serum IgE levels were high in ten of the 31 children with Giardia positive . In Giardia negatif group, total serum IgE levels were normal (p= 0.04). Skin prick test were performed in all of the patients. In Giardia positive group, there were 6 cases (19.3%) having skin prick test positivity and showing allergic reactions to Dp, grass pollens, Artemisia, and Salicea. In Giardia negatif group, there were no positive reactions. Within Giardia positive group, spesific IgE levels studied in 20 out of 31 children, 9 (45%) of them were class II or higher. Conclusion: The results revealed that there is a relationship between giardiasis and serum IgE response but not confirmed enhanced food allergy.

Amaç: Giardiazis morbiditesi ile tüm dünyada önemli bir halk sağlığı problemidir. Giardia infeksiyonu gastrointestinal epitelde geçirgenlik artışı ile besin allerjenlerinin emiliminde artışa ve besin allerjisine neden olabilir. Literatürde giardiazis ve atopi ilişkisi açık değildir. Çalışmamızın amacı giardiazis kliniğine benzer gastrointestinal belirtilerle başvurup giardia saptanan çocuklarda atopiyi deri testi ve spesifik IgE antikorları ile araştırmaktır. Gereç ve Yöntem: Hastaların dışkı örnekleri direkt floresan antijen (DFA) yöntemiyle incelendi. Standart bir forma hastaların sosyoekonomik durumu ve hijyen koşulları (ailenin aylık gelir düzeyi, yaşanılan yer ve içme suyu kaynağı) ve allerji öyküleri kaydedildi. İnek sütü, yumurta ve buğday unu allerjenleri ve genel aeroallerjenlerle prik yöntemle allerji deri testleri yapıldı. Total serum IgE ve inek sütü, yumurta ve buğday unu ile ev tozu akarları, mantarlar, ve ağaç, çayır, yabani ot polenleri için spesifik IgE ölçüldü. Bulgular: Gastrointestinal bulgusu olan 43 çocuğun (%65.2’si kız) dışkı DFA incelemesinde 31 (%72.1)’inde Giardia lamblia pozitifti. Giardia pozitiflerin 10 (%32.3)’unda total IgE yaşa göre normalden 2 standart sapma yüksek idi. Giardia negatif olan olguların ise, hiçbirinde serum total IgE düzeyi yüksek saptanmadı (p= 0.04). Deri testi tüm gruba yapıldı ve Giardia pozitiflerin 6 (%19.3)’sında Dp, Artemisia, Salicea, çayır allerjenleri pozitif idi. Giardia negatif grupta deri testi pozitifliği saptanmadı. Giardia pozitiflerin 20’sinde spesifik serum IgE çalışıldı ve 9 (%45)’unda bir veya daha fazla allerjene karşı class II ve üzerinde pozitif idi. Sonuç: Çalışmamızda giardiazisli çocuklarda total serum IgE yüksekliği vardı ve besin allerjisi ile ilişki saptanmadı.
