Intraocular pressure rise secondary to uveitis

Abstract

Purpose: To investigate the etiologies and clinical features of patients with intraocular pressure (IOP) rise secondary to uveitis. Materials and Methods: The data of 1314 patients who were followed in the Uvea - Behçet section between 2005 and 2011 were retrospectively evaluated and the medical records of 121 patients who had taken anti-glaucomatous agents because of secondary uveitic glaucoma or IOP rise were retrieved. Results: IOP rise or secondary glaucoma was detected in 121 (9.2%) patients with uveitis. Of these patients, 40 (33.1%) had Behçet's disease, 28 (23.1%) had herpetic uveitis, 18 (14.9%) had Fuchs' uveitis, 13 (10.7%) had idiopathic uveitis, 5 (4.2%) had HLA-B27 associated uveitis, 4 (3.3%) had pars planitis, 3 (2.5%) had ankylosing spondylitis, 2 (1.7%) had multifocal choroiditis, and 2 (2.5%) had Vogt Koyanagi Harada disease. Toxoplasmosis, toxocariasis, sarcoidosis, familial Mediterranean fever, Lyme disease, and serpiginous choroiditis were observed in 1 patient each (0.8%). Trabeculectomy with mitomycin C was performed in 12 (9.9%) patients and suprachoroidal seton implantation was performed in 5 (4.1%) patients. IOP could be controlled with surgical excision of sub-tenon triamcinolone acetonide deposits in a patient (0.8%) with intermediate uveitis who received a posterior sub-tenon injection of triamcinolone. Conclusion: IOP rise secondary to uveitis was observed in 9.2% of our cases and 14.8% of patients needed a surgical procedure. Behçet's disease was found to be the most commonly associated reason for IOP rise or glaucoma secondary to uveitis and the need for surgical treatment of glaucoma was higher in patients with Behçet's disease.

Amaç: Üveite sekonder göz içi basınç (GİB) artışı olan olguların etyolojik ve klinik özelliklerini araştırmak. Gereç ve Yöntemler: Hastanemiz Uvea-Behçet bölümünde, 2005-2011 tarihleri arasında takip edilen 1314 üveit olgusunun ve bunların içerisinde üveite sekonder GİB artışı nedeniyle antiglokomatöz ilaç kullanmış 121 olgunun tıbbi kayıtları geriye dönük olarak incelendi. Bulgular: 121 (%9.2) üveit olgusunda GİB artışı saptandı. Bu olguların üveit tiplerine bakıldığında; 40 (%33.1) olguda Behçet Hastalığı, 28 (%23.1) olguda herpes enfeksiyonu, 18 (%14.9) olguda Fuchs üveiti, 13 (%10.7) olguda idyopatik üveit, 5 (%4.2) olguda HLA B27 üveiti, 4 (%3.3) olguda pars planit, 3 (%2.5) olguda ankilozan spondilit, 2 (%1.7) olguda multifokal koroidit, 2 (%1.7) olguda Vogt Koyanagi Harada hastalığı, birer (%0.8) olguda ise, toxoplazma, toksokara, sarkoidoz, ailesel Akdeniz ateşi, Lyme hastalığı ve serpiginöz koroidopati bulundu. Glokomlu olgulardan 12'sinde (%9.9) mitomisinli trabekülektomi, 5 (%4.1) olguda ise suprakoroidal şant ameliyatı uygulandığı görüldü. İntermediate üveit nedeniyle uygulanan arka subtenon triamsinolon enjeksiyonu sonrası GİB kontrol altına alınamayan bir hastada (%0.8) ise görülebilen birikintilerin eksizyonu işlemi yapılmıştı. Sonuç: GİB artışı %9.2 üveit olgusunda tesbit edildi. Bunların %14.8'inde cerrahi müdahale gerekti. GİB artışının en sık görüldüğü ve glokom cerrahi tedavisine en çok gereksinim gösteren üveit tipi ise, Behçet hastalığı olarak saptandı.
