Clinical and molecular evaluation of symptomatic children with profound biotinidase deficiency in whom diagnosis is delayed

Abstract

Aim: Biotinidase deficiency is an autosomal recessive disorder of biotin metabolism . Biotin is the coenzyme of four carboxylases, important for mammals. Biotinidase recycles the biotin. Children with biotinidase deficiency become secondarily biotin deficient and neurologic and cutaneous symptoms occur. The objective of this study was to determine the difference in clinical and laboratory manifestations and the result of late onset therapy in symptomatic children with profound biotinidase deficiency with different mutations. Method: We report six symptomatic children (2 males, 4 females) with profound biotinidase deficiency (mean serum biotinidase activity 0.082±0.089 nmol/min/mL; mean normal activity is 6.62±1.77 nmol/min/mL) who exhibited early onset of symptoms (3.6±2.6 months, range:1-8 months). The serum biotinidase activity was determined by the colorimetric method and the diagnosis of profound biotinidase deficiency was confirmed in these children by demonstrating less than 10% of mean normal biotinidase activity in their serum. Clinical and laboratory findings of the patients, including neurologic examination, developmental tests, cranial imaging studies, tests to determine visual and hearing function were evaluated. Also the results of biotin therapy was determined. The polymerase chain reaction was used to amplify gene-spesific DNA of the biotinidase gene which were subsequently used for single-stranded confirmational analysis. Results: The diagnosis of biotinidase deficiency was established after the neurologic symptoms at a mean age of 4.5 months (range: 3-18 months). Clinical findings included coma, psychomotor retardation, seizures, skin rash, seborrheic dermatitis, partial alopecia, total alopecia, hypotonia or hypertonia, dyspnea, inspiratory stridor, recurrent infections and metabolic acidosis. Five patients were homozygous for the following mutations: R79C (n=2), a frameshift (98:del17ins3)(n=2) and Q456H (n=1) and one patient was compound heterozygous for the 3'splice site and V457M mutations. All the children, were treated with oral biotin shortly after the diagnosis was made. The clinical symptoms improved in all of the patients after biotin therapy but all had residual neurologic damage. The worst neurologic outcome was observed in patients who were homozygous for R79C mutation. Conclusion: The poor neurologic outcome of late diagnosed symptomatic patients indicate the necessity for newborn screening and early treatment of biotinidase deficiency.

Amaç: Biotinidaz eksikliği biotin metabolizmasının otozomal resesif bir bozukluğudur. Biotin, memeliler için önemli olan dört karboksilaz enzimin kofaktörüdür. Biotinidaz biotinin, biotin döngüsüne kazandırılmasını sağlar. Eksikliğinde gelişen sekonder biotin eksikliğine bağlı nörolojik bulgular ve deri bulguları görülür. Bu çalışmanın amacı, farklı mutasyonlar taşıyan semptomatik ağır biotinidaz eksikliği olan çocuklarda klinik ve laboratuvar farklılıkların saptanması ve geç tanılı vakalarda tedavi sonuçlarının değerlendirilmesidir. Yöntem: Ağır biotinidaz eksikliği olan (ortalama serum biotinidaz aktivitesi 0.082±0.089 nmol/dk./mL; ortalama normal aktivite 6.62±1.77 nmol/dk./mL) ve erken semptomatik olan (3.6±2.6 ay, 1-8 ay arası) altı vaka (2 erkek, 4 kız) sunuldu. Serum biotinidaz aktivitesi kolorimetrik metodla saptandı ve belirgin biotinidaz eksikliği tanısı biotinidaz enzim aktivitesinin ortalama normal enzim aktivitesinin %10'undan az olduğunun gösterilmesi ile kesinleştirildi. Hastaların klinik ve laboratuvar bulguları değerlendirildi. Nörolojik muayene, gelişim düzeyini belirlemeye yönelik testler, kraniyal görüntüleme, işitme ve görme işlevlerini değerlendiren testler uygulandı. Uygulanan biotin tedavisine yanıtları değerlendirildi. Biotinidaz geni için özgül DNA amplifikasyonu polimeraz zincir reaksiyonu ile sağlandı ve sonra "tek sarmal doğrulama polimorfizmi analizi" yapıldı. Bu şekilde tüm çocuklar ve ailelerinde saptanan mutasyonlar "otomatize DNA dizileri" ile doğrulandı. Bulgular: Biotinidaz eksikliği tanısı nörolojik semptomlar ortaya çıktıktan sonra ortalama olarak 4.5 ayda (3-18 ay arası) kondu. Gözlenen klinik bulgular koma, psikomotor retardasyon, konvülziyon, deri döküntüsü, seboreik dermatit, kısmi alopesi, tam alopesi, hipo ya da hipertoni, dispne, inspiratuar stridor, tekrarlayan enfeksiyonlar ve metabolik asidozdan oluşuyordu. Homozigot mutasyon taşıyan 5 hasta vardı: İkisi R79C, biri Q456H ve ikisi çerçeve kayması (98:del17ins3). Bir hasta 3' kırpılma bölgesi ve V457M mutasyonları için heterozigot idi. Teşhisten hemen sonra çocukların hepsi oral biotin ile tedavi edildi. Hepsinde klinik düzelme görülmesine rağmen kalıcı nörolojik hasar oluşmuştu. En ağır nörolojik hasar homozigot R79C mutasyonu taşıyan hastalarda gözlendi. Sonuç: Geç tanılı semptomatik çocuklarda nörolojik bulguların ağırlığı, biotinidaz eksikliği için yenidoğan taraması ve erken başlatılan tedavinin önemini artırmaktadır.
