A large solitary trichoepithelioma clinically confused with nodular basal cell carcinoma

Abstract
Objective: Trichoepithelioma is an uncommon benign skin tumor that shows follicular differentiation. The diagnostic dilemma in differentiating solitary trichoepitheliomas from basal cell carcinoma (BCC) is well documented. This margin of error becomes important when its impact on patient management is taken into account. Case report: A 75-year-old female patient applied with a nodular lesion located on the lateral aspect of the right ala of her nose, measuring 2x1.7x1 cm, with central ulceration and crusts. A clinical diagnosis of nodular basal cell carcinoma was considered. The lesion was excised with a surgical margin of 4 mm under general anesthesia. The resulting defect was closed with a superiorly based nasolabial flap. The histopathologic diagnosis of the excised specimen was reported to be trichoepithelioma. Conclusion: Sometimes it is not possible to clinically discriminate trichoepitheliomas from nodular BCCs. Solitary trichoepithelioma should be taken into account in the differential diagnosis of nodular BCCs located in the head and neck region. Radical surgical excisions may be prevented with incisional biopsy before surgical therapy.

Amaç: Trikoepitelyoma folliküler diferansiyasyon gösteren ve ender görülen bir deri tümörüdür. Soliter trikoepitelyomaların bazal hücreli karsinomadan ayırımının zorluğu iyi bilinmektedir. Bu hata payı hastanın tedavisi söz konusu olduğunda önem arz etmektedir. Olgu sunumu: Yetmiş beş yaşında bayan hasta burun sağ kanadı lateralinde yerleşmiş, 2x1,7x1 cm boyutlarında, santral olarak ülserasyon ve kurut içeren nodüler lezyon nedeniyle başvurdu. Klinik olarak nodüler BCC tanısı düşünüldü. Lezyon genel anestezi altında 4 mm cerrahi sınırla eksize edildi. Oluşan defekt superior yerleşimli nazolabial fleple tamir edildi. Cerrahi materyalin histopatolojik tanısı trikoepitelyoma olarak rapor edildi. Sonuç: Trikoepitelyomaları nodüler BCC’den klinik olarak ayırt etmek bazen mümkün olmayabilir. Baş-boyun bölgesinde yerleşen nodüler BCC’lerin ayırıcı tanısında trikoepitelyoma da düşünülmelidir. Cerrahi tedavi öncesi yapılacak insizyonel biyopsi ile daha radikal cerrahi tedaviler önlenebilir.
