The effects propofol and dexmedetomidine infusions on sedation and haemodynamic parameters during surgical interventions under brachial plexus anesthesia

Abstract
Objective: The aim of this study was to investigate the effects of propofol and dexmedetomidine infusions on sedation levels and haemodynamic parameters during surgical interventions under brachial plexus anesthesia. Method: Forty patients scheduled for elective hand and forearm surgery were randomized equally to receive either propofol (1 mg kg-1 loading dose; maintenance 50 µg kg-1 min-1 ) or dexmedetomidine (1 µg kg-1 initial loading dose for 10 min; maintenance 0.5 µg kg-1 h-1 ). Haemodynamic parameters (mean arterial blood pressure, heart rate), ventilation (respiratory rate, peripheric oxygen saturation) and sedation (Ramsay and modified Observer’s Assessment of Alertness and Sedation) were determined at 5, 10, 20, 30, 40, 50 and 60 minutes during surgery. The patient and surgeon satisfaction scores and the first analgesic requirement times of the two groups were recorded and compared. Results: The propofol and dexmedetomidine infusions resulted in lower blood pressure and heart rate, respectively, compared to the baseline and the other group. SpO2 was significantly reduced in the propofol group after the loading dose. Sedation was achieved more rapidly with propofol, but was similar between the two groups 30 minutes after the loading dose. The first analgesic requirement time was approximately 3 times longer in the dexmedetomidine group. Conclusion: Dexmedetomidine achieved more respiratory stability and similar levels of sedation to propofol, albeit with a slower onset of sedation. Thus, dexmedetomidine may prove to be a useful adjuvant for surgery performed under regional anesthesia, especially when postoperative pain might be predicted to be worse than usual.

Amaç: Brakiyal pleksus anestezisi altında, el ve önkol cerrahisi geçirecek hastalarda, propofol ve deksmedetomidin infüzyonunun sedasyon düzeyleri ve hemodinami üzerine etkilerinin araştırılması amaçlanmıştır. Yöntem: Çalışmaya, brakiyal pleksus anestezisi altında cerrahi planlanan 40 hasta dahil edildi. Grup P’ye 1 mg kg-1 yükleme dozunu takiben, 50 µg kg-1 dk-1 dozunda propofol infüzyonu yapılırken; Grup D’ye 1 µg kg-1 yükleme dozu 10 dakikada verildikten sonra, 0.5 µg kg-1 st -1 dozunda deksmedetomidin infüzyonu yapıldı. Yükleme dozunu takiben 5, 10, 20, 30, 40, 50 ve 60. dk’larda ortalama arteriyel basınç (OAB), kalp atım hızı (KAH), periferik oksijen satürasyonu (SpO2), solunum sayısı (SS), Ramsay ve modifiye OAA/S (Observer’s Assessment of Alertness and Sedation Scale) skorları kaydedildi. Ayrıca hasta ve cerrah memnuniyeti ile ilk analjezik gereksinimi de kaydedilerek gruplar arasında karşılaştırıldı. Bulgular: Propofol grubunda OAB, deksmedetomidin grubunda ise KAH başlangıca ve diğer gruba göre düşük bulundu. SpO2 ise yalnızca propofol grubunda, özellikle yükleme dozunu takiben başlangıca göre anlamlı olarak düşüktü. Propofolle başlangıçta daha hızlı ve derin bir sedasyon sağlanırken, 30. dk’dan sonra gruplar arasında sedasyon skorları benzerdi. ‹lk analjezik gereksinim zamanı deksmedetomidin grubunda yaklaşık 3 kat uzun bulundu. Sonuç: Deksmedetomidin daha yavaş başlangıçlı olmakla birlikte, propofole benzer sedasyon düzeyleri sağlayabildiğinden ve hastaların solunum parametrelerini daha iyi koruduğundan, özellikle postoperatif ağrının belirgin olacağı, rejyonal anestezi uygulamalarında, propofole alternatif olarak sedasyon için tercih edilebilir.
