Anesthetic management in patients with samter triad

Abstract
Background: The incidence of the triad defined as Samter's Triad, or aspirin induced asthma (AIA), including bronchial asthma, nasal poliposis and analgesic intolerance is 1 %. In this retrospective study, we aimed to share and discuss our experiences in 54 patients with AIA under general anaesthesia. Method: Of the patients diagnosed with Samter's Triad after medical or surgical treatment and being followed up in the allergy unit of Department of Chest Diseases, between February 1998 and June 2006, fifty-four patients who were operated on because of ear-nose-throat problems were included in the study. Demographic data, types of surgery, preoperative evaluation, postoperative status, medications and methods of anesthesia were reported. Results: Most of the surgical procedures, a total of 61 procedures in 54 patients, were endoscopic sinus surgery. While there was advanced asthma in one third of the patients, the others showed moderate or mild asthma. In our study, 67% of the patients were using steroids, 83% were using Salbutamole nebules; totally 87% of the patients were taking medications due to recent illnesses and 3% were not receiving any medical treatment. In 12 patients, the provocation test was positive. Postoperative endotracheal re-entubation was required in 2 patients because of bronchospasm. Conclusion: It is important for preoperative evaluation and preparation of AIA patients to be done in collaboration with allergy and anesthesiology clinics and for the most suitable anesthetic approach to be chosen in all types of surgical procedures. According to our anesthesia protocol, induction made with propofol, fentanyl, or vecuronium and maintained with sevoflurane or isoflurane,with fentanyl given for postoperative analgesia, seem to be a safe anesthetic approach.

Amaç: Samter Triadı ya da aspirinin indüklediği astım (AIA) olarak tanımlanan ve nazal poliposis, bronşiyal astım ve analjezik intoleransını içeren triadın görülme sıklığı %1'dir. Bu retrospektif çalışmada, genel anestezi alan AIA'lı 54 hastayı tartışmayı ve deneyimlerimizi paylaşmayı amaçladık. Yöntem: Göğüs hastalıkları Allerji Ünitesinde AIA tanısı ile izlenen tıbbi ve cerrahi tedavi almış olan hastalardan Şubat 1998 ile Haziran 2006 tarihleri arasında Kulak Burun Boğaz bölümünce cerrahiye alınan 54 hasta çalışmaya dahil edildi. Hastaların demografik verileri, cerrahi tipleri, preoperatif değerlendirmeleri, postoperatif durumları, astım hikayesinin süresi ve derecesi, aldıkları tıbbi tedavi, anestezi yöntemleri hasta dosyaları ve anestezi formlarından kaydedildi. Bulgular: Elli dört hastaya en çok endoskopik sinüs cerrahisi olmak üzere 61 cerrahi işlem uygulandı. Hastaların 113'ünün yüksek, 213'ünün orta ve düşük derecelerde astım kliniği vardı..Çalışmamıza alınan hastalardan %67'si steroid, %83'ü Salbutamal nebül, %87'si eşlik eden diğer hastalıkları nedeniyle tıbbi tedavi alıyor, %3'ü herhangi bir ek tedavi almıyordu. 54 hastanın 12'sinde oral provokasyon testi pozitifti. Hastaların ikisinde postoperatif bronkospazm nedeniyle tekrar entübasyon gerekti. Sonuç: Allerji ve Anesteziyoloji Klinikleri tarafından AIA hastalarının preoperatif değerlendirilmesi ve hazırlanması, uygulanılacak tüm cerrahi girişimler için uygun anestezik yaklaşımın belirlenmesi açısından önemlidir. Deneyimlerimiz; indüksiyonda propofol, fentanil, veküronyumun idamede sevofluran veya izofluranın, postoperatif analjezi için ise fentanilin güvenle kullanılabileceğini düşündürmektedir.
