Escobar syndrome (multiple pterygium syndrome) associeted with thoracic kyphoscoliosis: Two siblings

Abstract

Purpose: Escobar syndrome (ES) is characterized by a web across every flexion crease in the extremities, most notably the popliteal space. The syndrome is associated with other structural anomalies: a vertical talus, clubfoot, thoracic kyphoscoliosis and severe restrictive lung disease. In our study we evaluated two brothers diagnosed with Multiple Pterygium Syndrome type Escobar. The purpose of this study was to assess to characterize the abnormalities of the vertebrae and concomitant anomalies, to assess the results of the treatment. Cases report: Two brothers (age 11 and 15) were diagnosed with ES via genetic analysis. The younger brother was diagnosed with kyphosis and progressive scoliosis, high palate, ptosis, low-set ear, arachnodactily, faciocranial dimorphisms, mild deafness, clubfoot and joint contractures. The older brother had the same symptoms and but was not scoliosis. Both received operations for inguinal hernia, clubfoot correction, dislocated hip in early childhood. Cobb method was used to measure spine deformity. Both patients underwent complete magnetic resonance imaging (MRI) evaluation of the spinal canal. The younger patient received posterior vertebral instrumentation between thoracic 3 – lumbar 1 (T3- L1). At final follow-up, patients were evaluated for coronal/sagittal plane correction, implant failure and satisfaction. Results: Simple posterior stabilization of the deformity was achieved in the younger brother. The younger patient had a scoliosis curve (78º) and thoracic kyphosis (70º). Postoperatively correction rate was 42.3 % (33º). A follow-up period of 4 years revealed that the correction rate was maintained but the patient developed proximal kyphosis and a scoliosis curve progression in the lumbar area. The older patient was diagnosed with 83º kyphosis. For the younger patient MRI revealed L5 was sacralized, cauda equine level was at L3 and proximal thoracic fusion and the older patient was diagnosed with syringomelia and proximal thoracic fusion. Conclusions: Spinal pathologies commonly occur in patients with ES. Scoliosis may progress considerably over time. Early surgical treatment is recommended. However junctional kyphosis and/or progressive scoliosis in nonfusion areas can develop after corrective surgery.

Özet

Amaç: Escobar sendromu (ES) ekstremitelerde belirgin olarak popliteal bölge gibi katlanma bölgelerinde ağ şeklinde cilt yapıları ve eklem kontraktürleri ile karakterize, vertikal talus, pes ekinovarus (PEV) torasik kifoskolyoz ve ciddi restriktif akciğer hastalığı gibi diğer yapısal anomalilerinde eşlik ettiği bir sendromdur. Çalışmamızda Multipl Pterygium Sendrom (MPS) nun Escobar tipi tanısı konulan 2 erkek kardeşte birlikte görülen vertebra ve diğer anomaliler ile tedavi sonuçlarının değerlendirilmesi amaçlanmıştır. Olgu sunumu: Genetik inceleme sonucu ES tanısı konulan 2 erkek kardeş (11 ve 15 yaş) omurga problemleri ve diğer ortopedik sorunları açısından değerlendirildi. Genç kardeşte (olgu-1); kifoz, ilerleyici skolyoz, yüksek damak, pitoz, düşük kulak kepçesi, araknodaktili fasiokranial disrafizm, hafif işitme kaybı, vertikal talus ve yaygın eklem kontraktürleri vardı. Büyük kardeşde (olgu-2) aynı bulgular yanında, skolyoz yoktu. Her iki kardeş erken çocukluk dönemlerinde diğer patolojileri için çeşitli operasyonlar geçirmişti. Omurga eğrilikleri Cobb metodu ile ölçüldü. İntraspinal anomalilerin araştırılması için manyetik rezonans görüntüleme (MRG) yöntemi kullanıldı. Olgu-1 de ilerleyici skolyoz deformitesi için torokal 3 - lomber 1 (T3- L1) vertebralar arasına posterior spinal korreksiyon ve füzyon uygulandı. Olgu-2 de omurga problemleri için herhangi bir müdahele yapılmadı. Her iki kardeşte omurga ve diğer ekstremite patolojileri açısından 4 yıl takip edildi. Sonuçlar: Olgu-1ʼde preoperatif skolyoz eğriliği 78º ve torasik kifozu 70º idi. Postoperatif erken dönemde 33º ye geriledi (düzelme % 42.3). Omurgasında L5 vertebra sakralize ve proksimal torasik füzyon mevcuttu. MRG ile; kauda equina L3 seviyesinde (tethered kord) görüntülendi. Postoperatif yaklaşık 4 yıl takip edilen hastada implant proksimalinde kifoz, distalinde ise lomber bölgede skolyoz eğriliğinde artış oldu. İmplant proksimalindeki kifoz ile birlikte gelişen cilt problemleri nedeniyle füzyon olduktan sonra postoperatif 3. yılında implantları başka bir merkezde çıkarıldı. Postoperatif özellikle koronal dengesinin sağlandığı gözlemlendi. Olgu-2 de üst torasik bölgede belirgin 83º kifoz ölçüldü. Proksimal torasik füzyon ile birlikte MRG de siringomyeli tespit edildi. Omurga problemleri için herhangi bir işlem uygulanmadan gözlemlendi. Her iki kardeşte takip süresince bağımsız olarak okullarına gidebildi ve yaşamlarını sürdürdü. Sonuç: ES ile birlikte omurga patolojileri sık görülür. İntraspinal anomaliler eşlik edebilir. Özellikle erken progresyon göstermesi nedeniyle skolyoz deformitesinin cerrahi tedavisi hastanın yaşam kalitesini korumak ve düzeltmek için önerilir. Ancak özellikle füzyon sahası dışında kifoz gelişimine ve skolyoz ilerlemesine dikkat edilmelidir.
