Effect of needle size on the diagnostic adequacy in fine needle aspiration of thyroid nodules

Abstract
Objective: To evaluate the effect of needle size on diagnostic adequacy of the specimens in fine needle aspiration of thyroid nodules. Methods: In our institution, various needle sizes have been used in fine needle aspiration of thyroid nodules, ranging from 21 G to 25 G. In this study, we have retrospectively compared the adequacy rates of biopsy specimens in two age-and-sex matched groups of patients. The groups consisted of patients in whom the specimens were collected by the same radiologist, using the same biopsy technique (4-5-core biopsies), however with different needle sizes (21 G or 25 G) in each group. Only the solid nodules or nodules with solid component, with diameters exceeding 1 cm were included in the study. Aspirates with cystic content were not included in the analyses. Results: Statistical analysis revealed that rates of specimen adequacy were significantly higher (p=0.002) in biopsies performed with larger (25 G) needles (22.5% vs. 40.7% in 21 G and 25 G groups, respectively). Conclusion: To our knowledge, this is the first study to show that needle size may have an impact on the adequacy rates of the specimens in fine needle aspiration of thyroid nodules. Results are discussed with suggestions to improve the biopsy results performed with 25 G needles.

Amaç: Tiroit nodüllerinin ince iğne aspirasyon biyopsisinde iğne çapının spesimen yeterliliği üzerindeki etkisini değerlendirmek. Yöntemler: Kurumumuzun geçmişinde tiroit nodüllerinin ince iğne aspirasyon biyopsisi için 21 Gden 25 Gye kadar değişen farklı iğne çapları kullanılmıştır. Bu çalışmada, yaş ve cinsiyet denkliği sağlanan iki grup hastada biyopsi yeterliliği oranları araştırılmıştır. Gruplar aynı radyolog tarafından ve aynı teknik kullanarak (nodüllerden 4-5 iğne geçişi yapmak suretiyle), ancak her bir grupta farklı iğne çapları kullanılan (21 G veya 25 G) hastalardan oluşmaktaydı. Çalışmaya çapı 1 cmyi aşan solid veya solid bileşeni bulunan nodüller dâhil edildi. Kistik içerik aspirasyonu olan nodüller çalışma dışı bırakıldı. Bulgular: İstatistiksel analiz sonucunda kalın iğnelerle yapılan biyopsilerde spesimen yeterliliği oranı belirgin olarak (p=0,002) yüksek bulundu. Bu oran 21 G grubunda %22,5, 25 G grubunda %40,7 idi. Sonuç: Bildiğimiz kadarıyla bu çalışma tiroit nodüllerinin ince iğne aspirasyon biyopsisinde iğne çapının spesimen yeterliliği üzerindeki etkisini değerlendiren ilk çalışmadır. Sonuçlar tartışılmış, 25 G iğnelerle yapılan biyopsi sonuçlarının iyileştirilmesine yönelik öneriler sunulmuştur.
