Ex-Press Mini Glaucoma Shunt Implantation with 5-Fluorouracil in Pseudophacic Open Angle Glaucoma Patients

Abstract

Purpose: To evaluate the outcomes of Ex-press mini glaucoma shunt implantation with 5-Fluorouracil (5-FU) in pseudophacic OAG ( open angle glaucoma) patients.Material and Methods: We assessed retrospectively 11 eyes of the 8 OAG patients whom had been operated with uncomplicated phacoemulsification surgery previously, and have high IOP despite maximal tolerable medication and then operated with ex-press shunt surgery with 5-Fluorouracil.Results: Cases are followed on average 15.3±7.5 months after the surgery. Mean IOP (intraocular pressure) of 11 eyes was detected as 27.9+-3.2 mmHg pre-operatively and reduced to 13.6+/-2.2 mm Hg post-operatively at the last control. Decrease in the IOP was statistically significant (p<0.05). Average number of pre-operative medications was 2.63±0.5 and none of the cases needed medical treatment post-operatively. We did not have any complication except cystic bleb occurrence in one patient. No significant change was recorded in visual acuity and c/d ratios.Conclusions: Although we have limited number of cases, Ex-press implantation with 5-Fluororuracil was found quite effective and safe in control of IOP in pseudophacic OAG patients. Few studies are present about Ex-press implantation in these pseudophacic patients and we think that firstly usage of 5-Fluorouracil will provide additional contribution to literature

Amaç: Psödofak açık açılı glokomu (AAG) olan olgularda 5-Fluorourasil (5-FU) ile Ex-press mini glokom şant implantasyonu ameliyatı sonuçlarının değerlendirilmesi amaçlandı.Gereç ve Yöntem: Fakoemülsifikasyon yöntemiyle komplikasyonsuz katarakt cerrahisi geçirmiş, tolere edilebilir maximum medikal tedaviye rağmen göz içi basıncı (GİB) kontrolü sağlanamaması nedeniyle 5-FU ile Ex-press mini glokom şant implantasyonu uygulanmış ileri evre 8 AAG'lu olgunun 11 gözüne ait kayıtlar geriye dönük olarak incelendi.Bulgular: Olgular ameliyat sonrası ortalama 15.3±7.5 (6-26 ay) süreyle takip edildiler. Ameliyat öncesi GİB ortalaması 27.9±3.2 mmHg olan olguların ameliyat sonrası son kontrol muayene GİB ortalaması 13.6±2.2 mmHg olarak tespit edildi. GİB'ndaki düşüş istatistiksel olarak anlamlıydı (p<0.05). Ameliyat öncesi ilaç sayısı ortalaması 2.63±0.5 iken ameliyat sonrası hiçbir gözde ilaç gereksinimi olmadı. Bir olguda görülen kistik blep dışında herhangi bir komplikasyonla karşılaşılmadı. Ameliyat sonrası olguların görme keskinlikleri ve c/d oranlarında anlamlı değişim gözlenmedi.Sonuç: Olgu sayımız sınırlı olmakla birlikte psödofakik AAG'lu olgularda 5-FU ile Ex-press şant implantasyonu GİB kontrolünde etkin ve güvenilir bulundu. Bu olgu grubunda Ex-press şant implantasyonu ile ilgili sınırlı sayıda rapor olup, çalışmamızın 5-FU ile birlikteliği açısından da literatüre ek katkı sağlayacağı düşüncesindeyiz
