Intranasal foreign body without nasal discharge presenting with halitosis

Abstract

Halitosis is a social and medical problem in both pediatric and adult age groups. Poor oral hygiene and dental infections are the leading causes of halitosis. Although nasal foreign body is a frequent diagnosis in childhood, it’s rarely seen as a causative factor for halitosis without unilateral foul smelling nasal discharge. In this case report; a 4 year old child, who suffered from halitosis for 5.5 months despite several treatment attempts, is presented. Nasal foreign body was identified to be the cause of halitosis. Because of complete obstruction of nasal cavity due to a piece of plastic bag, no unilateral, purulent nasal discharge was seen. Halitosis was caused by posteriorly draining nasal discharge. Nasal foreign body should be considered among the childhood halitosis cases.

Özet

Halitozis; gerek erişkin, gerekse çocuk yaş grubunda önemli bir sosyal ve tıbbi sorundur. Nedenleri arasında kötü ağız bakımı ve diş hastalıkları başta gelmektedir. Çocukluk yaş grubunda burunda yabancı cisim sıklıkla tek taraflı pürülan burun akıntısı ile belirti verir ve halitozise nadiren neden olmaktadır. Bu yazıda 5.5 aydır kötü ağız kokusu nedeniyle birçok farklı tedavi almasına rağmen fayda görmeyen ve halitozis etkeni olarak burunda yabancı cisim saptanan 4 yaşında bir çocuk hasta sunulmaktadır. Bu olguda bir naylon torba parçasının, nazal kavitede tam obstrüksiyon yaratarak burundan akıntı olmasını engellediği ve posteriora drene olan pürülan akıntının halitozise neden olduğu saptanmıştır. Çocuk yaş grubu halitozis vakalarında burunda yabancı cismin mutlaka akla getirilmesi gereğini vurgulamak üzere bu olgu sunulmuştur.
