Analysis of complications related to surgical treatment of lenke type 3 adolescence idiopathic scoliosis

Abstract

We aim to analyze the complications related to surgical treatment of Lenke Type 3 adolescence scoliosis with the 3rd generation posterior segmental instrumentation system retrospectively and compare with the data of literature. 21 of the patients (80.7 %) was female, 5 of the patients was male (19.3 %). The age of the patients was between 10 and 19 in the time of surgery and the mean age was 15 ± 2. The mean follow-up time was 60 months (19-94 months). Coronal deformities, kyphosis (T5-T12), lordosis (L1-L5) and correction ratios measured in standing-orthoroentgrams with Cobb method pre and postoperatively. Balance in coronal and sagittal planes was evaluated. The patient questionnaire of Scoliosis Research Society (SRS-30) was filled in for all patients. Peroperative and postoperative complications are divided into three classifications (early, technically and late complications) by when they had seen and their reasons. We observed 2 superficial wound infection (7,6 %), 1 (3 %) pneumothorax, 1 (3 %) minimal pleural effusion, 4 (15 %) pedicle fracture, 3 (11 %) dural injury when placing the screw, 1 (3 %) unilateral implant failure in level of L4-L5, 1 (3 %) coronal decompenzation while neural deficit, pseudoarthrosis, loss of lordosis and late infection was not observed in any patients. We conclude that surgical treatment of Lenke type 3 adolescence idiopathic scoliosis patients treated with 3rd generation instrumentation had low complication rates and reliable compared with literature.

Bu çalışmada, Lenke Tip 3 Adölesan İdiopatik Skolyoz (AİS) tanısı ile 3. kuşak posterior segmental enstrümantasyon sistemi kullanarak cerrahi tedavi uyguladığımız hastalarda gelişen komplikasyonları literatür verileriyle karşılaştırarak geriye dönük olarak analiz etmeyi amaçladık. Olguların 21’i (% 80,7) kadın, 5’ sı (% 19,3) erkek, ameliyat sırasında vakaların yaşları 10 ile 19 arasında ve ortalama yaş 15 ± 2 idi. Ortalama takip süresinin 60 ay (dağılım 19-94 ay) olduğu çalışma grubumuzdaki hastaların ameliyat öncesi ve sonrası çekilen ayakta ortoradyografilerde Cobb yöntemi ile koronal eğrilikleri, kifoz (T5-T12), lordoz (L1-L5) ve korreksiyon oranları ölçüldü. Koronal ve sagittal planda denge değerlendirildi. Tüm hastalar için Skolyoz Araştırma Cemiyeti’nin hazırlamış olduğu SRS-30 (Scoliosis Research Society-30) formu doldurdu. Ameliyat içi ve sonrası dönemde komplikasyonlar, ortaya çıkış zamanı ve sebeplerine göre erken, tekniğe bağlı ve geç komplikasyonlar olarak 3 sınıfa ayrıldı. Olgulardan 2’sinde (% 7,6) yüzeyel yara yeri enfeksiyonu, 1 (% 3) pnömotoraks, 1 (% 3) hafif düzeyde plevral efüzyon, 4 (% 15) pedikül kırığı, 3(% 11) vida yerleştirilmesi sırasında dural hasar, 1 (% 3) olgunun son kontrolünde lomber bölgedeki L4-L5 arasında unilateral implant kırılması, bir (% 3) hastada koronal dekompanzasyon görülürken, hiçbir vakada nörolojik defisit, psödoartroz, lordoz kaybı ve geç enfeksiyon görülmedi. Literatürdeki sonuçlar ile karşılaştırıldığında üçüncü kuşak enstrümantasyon sistemleri kullanılarak cerrahi tedavi uygulanan Lenke Tip 3 Adölesan İdiopatik Skolyozun komplikasyon oranlarının düşük ve güvenilir olduğunu düşünüyoruz.
