Determination of sensivity of Mycobacterium tuberculosis strains to isoniazid, rifampin streptomcycin and ethambutol by BACTEC system

Abstract
The sensitivity to 4 major antituberculous drugs of 99 out of 127 mycobacterial strains isolated in BACTEC 12B and Löwenstein-Jensen media from 1444 specimens collected in Mersin and vicinity in a one year period was determined by BACTEC 460TB system. Eighty strains were found susceptible to all drugs. Ten strains were resistant to only one of the drugs [5 for isoniazid (INH), 4 for rifampin (RIF), lfor streptomycin (STR), none for ethambutol (ETB)]. The strains resistant to two drugs were 3 (1 for each pair oflNH+RIF, RIF+ETB and INH+STR). One of the strains resistant to 3 drugs was resistant to INH+RIF+STR, 2 strains to INH+RIF+ETB and 1 strain to RIF+STR+ETB. Two strains were found resistant to all of 4 drugs. RIF resistance is accepted as the most important factor in unsuccessful antituberculous therapy, and we found it in 12 of the 99 strains with accompanied INH resistance in 6 strains. This shows that tuberculosis has a great importance for public health in our region and urgent actions should be taken for this problem.

Bir yıl içinde Mersin ve çevre bölgelerden gönderilen 1444 örnekten BACTEC 12B ve Löwenstein-Jensen besiyerlerinde yapılan kültürlerden 127 (% 8.8) mikobakteri susu üretilmiş, bunların 99'unun BACTEC 460TB sistemi ile 4 majör antitüberküloz ilaca duyarlılıkları belirlenmiştir. Suşların 80'i bütün ilaçlara duyarlı bulunmuştur. Bir ilaca dirençli 10 sustan 5'i izoniazide (İNH), 4'ü rifampisine (RİF), 1 'i streptomisine (STR) dirençli bulunmuş, yalnız etambutole (ETB) dirençli olan susa rastlanmamıştır. İki ilaca dirençli 3 susta birerinde İNH+RİF'e, RİF+ETB'e, İNH+STR'e direnç saptanmıştır. Üç ilaca dirençli 4 sustan l'i İNH+RİF+STR 'e, 2 'si İNH+RİF+ETB 'e, 1 'i RİF+STR+ETB 'e; 2 suş ise 4 ilaca da dirençli bulunmuştur. İNH+RİF direnci birlikteliğine 6 susta rastlanmıştır. Tüberküloz tedavisinde başarıyı engelleyen en önemli faktör olarak kabul edilen RİF direncine 99 sustan 12'sinde rastlanması, bunların 6'sında İNH direncinin de bulunması bölgemizde tüberkülozun çok önemli bir halk sağlığı problemi olduğunu göstermekte, acil önlemler alınması gerektiğine işaret etmektedir.
