The anesthetic management of a patient with achondroplasia

Abstract

Skeletal dysplasias are characterized by abnormalities of cartilage and bone growth. Achondroplasia is the most common nonlethal skeletal dysplasia and it is also the most common cause of dwarfism. The estimated frequency of achondroplasia is between 1/15,000 and 1/40,000 live births. The characteristics are a large protruding forehead, large head, depressed nasal bridge, short maxilla, large mandible, large tongue, tonsil enlargement, nasal/pharyngeal/laryngeal hypoplasia, thoracolumbar kyphosis and lumbar hyperlordosis. Neurologic dysfunction due to cervicomedullary compression, thoracolumbar spinal stenosis and scoliosis can also be seen in these patients. Obstructive sleep apnoea (OSA) is prevalent in a large percentage of patients with acondroplasia. The most important breathing disorder during sleep in these patients is upper airway obstruction. Adenoidectomy is an effective treatment for upper airway obstruction and using bilateral tympanostomy tubes is effective for otitis media with effusion. The airway and anesthetic management of these patients poses a significant challenge mainly because of associated anatomic difficulties. In this report, we discuss the anesthetic management of a patient with achondroplasia who underwent adenoidectomy and bilateral ear surgery.

Özet

İskelet displazileri anormal kartilaj ve kemik büyümesiyle karakterizedir. Akondroplazi en yaygın, letal olmayan iskelet displazisidir ve cüceliğin de en yaygın sebeplerinden biridir. Görülme sıklığı canlı doğumlarda 1/15.000 ile 1/40.000 arasında değişmektedir. Hastalığın karakteristik özellikleri geniş öne doğru bombeleşen alın, büyük kafa, basık burun kökü, kısa maksilla, geniş man- dibula, büyük dil, geniş tonsil, nazal/faringeal/laringeal hipoplazi, torakolumbar kifoz ve lumbar hiperlordozdur. Bu hastalarda servikomedüller kompresyona, torakolumbar spinal stenoza ve skolyoza bağlı nörolojik disfonksiyonlar da görülebilmektedir. Obstrüktif uyku apnesi akondroplazili hastaların çoğunda vardır. Bu hastaların uyku sırasındaki en önemli solunum problemi üst havayolu obstrüksiyonudur. Adenoidektomi üst havayolu obstrüksiyonu olan hastalarda uygulanması gereken tedavidir. Ventilasyon tüp takılması da, posterior nazofarenksin obstrüksiyonu sebebiyle sık seröz otit gelişen hastalarda etkin bir tedavi yöntemidir. Ancak akondroplazili hastalara cerrahi uygulamak, anatomik deformiteler nedeniyle zor hava yolu yönetimini ve zor anestezi uygulamasını da beraberinde getirmektedir. Bu olgu sunumunda, adenoidektomi ve bilateral ventilasyon tüpü uygulaması yapılan akondroplazili hastadaki anestezi deneyimimiz tartışılmıştır
