Comparison of the E_Ü'ects of Colloid and Crystalloid Priming Solutions on the Fluid Balance in On-Pump Open Heart Surgery

Abstract

Objective: There is no definitive suggestion for the con— tent of the priming solution in on pump open hearth surgery, but studies comparing the advantages and di— sadvantages ofpriming liquids are still ongoing. In our study, we aimed to compare the effects of colloid and crystalloid priming solutions on the final fluid balance in open heart surgery. Material and Method: Data from 100 patients with ASA I—II who underwent elective on pump open heart surgery were obtained from anesthesia files and pa— tient records. Hundred patients were included in the study, and data of 50 patients for whom crystalloid colloid priming solutions and 50 who received only crystalloid priming solutions were evaluated. Results: Fluid balance at the end of CPB was statis— tically significantly higher in the crystalloid group, while at the end of the surgery it was similar in both groups (p<0.001, p=0.09). The amount ofbloodpro— ducts used, and urine output estimated at the end of surgery were similar in both groups. Creatinine levels in the preoperative and postoperative periods were similar within, and between groups. Conclusion: HES 6% (130/04) decreasedfluid balance at the end ofCPB in on—pump open hearth surgery and this effect continued until the end ofsurgery. Effects on hemostasis and bleeding were similar with crystalloids. Although (HES) 6% (130/0.4) does not impair creati— nine levels in patients with normal renal functions, it should be contemplated as real drugs and used in app— ropriate doses andfor suitable durations

Özet

Amaç: On pump açık kalp cerrahisinde priming solüsyonu olarak seçilecek sıvılarla ilgili kesin bir öneri bulunmamakta, priming sıvılarının avantaj ve dezavantajlarını karşılaştıran çalışmalar halen devam etmektedir. Çalışmamızda, priming solüsyonu olarak kolloid+kristaloid kullanımıyla, yalnızca kristaloid kullanımının cerrahi sonu sıvı dengesi üzerine etkisini karşılaştırmayı amaçladık. Gereç ve Yöntem: ASA I-II on-pump elektif açık kalp cerrahisi geçiren toplam 100 hastaya ait veri hasta dosyası ve anestezi formlarından elde edildi. Priming solüsyonu olarak kristalloid+kolloid kullanılan 50 hasta ile, priming solüyonu olarak yalnızca kristaloid kullanılan 50 hastanın verileri değerlendirildi.Bulgular: KPB sonunda sıvı dengesi kristaloid grubunda istatistiksel olarak anlamlı derecede fazla iken, cerrahi sonunda 2 grupta da benzer bulundu, (p<0.001, p:0.09). Cerrahi sonunda kullanılan kan ürünü miktarı ve idrar çıkışları her 2 grupta benzerdi. Kreatinin değerleri preoparatif ve postoperatif dönemlerde hem gruplar arasında hem de grup içinde benzer seyretti.Sonuç: %6 HES (130/0.4), on-pump açık kalp cerrahisinde KPB sonucu sıvı dengesini azaltmakla birlikte, bu etkisi cerrahinin sonuna kadar devam edebilmiştir. Kanama ve hemostaz üzerine etkisi kristaloidlerle benzer bulunmuştur. Yüzde 6 HES (130/0.4)'ün böbrek fonksiyonları sağlam olan hastalarda kreatinin düzeylerini olumsuz etkisi görülmemekle birlikte, gerçek ilaçlar gibi düşünülüp uygun hastalarda uygun doz ve sürede kullanılmalıdır
