Abstract

Introduction: Atypical meningiomas represent 20% of all meningiomas. The aim of this retrospective study is to analyze 24 cases of atypical meningiomas following surgical resection at our clinic. Material and Method: 24 cases who were operated at İzmir Bozyaka Training and Research Hospital Neurosurgery Clinic and given a diagnosis of atypical meningioma in the period from 2013 to 2017 are included to our study. Patient age, gender, presenting symptoms, tumor location, Simpson resection grade, postoperative complications and recurrences are evaluated. Results: 24 (25%) of 98 cases who had operated in our clinic due to intracranial meningioma were diagnosed as atypical meningioma. There were 12 male patients and 12 female patients. Ages ranged from 24-71 years, and mean age was 55,87. The most common presenting clinical manifestations were symptoms of increased intracranial tension in 16 patients (67%) and neurologic symptoms in 7 patients (29%). The predominant site of atypical meningioma was convexity and parasagittal/ falx (63%) and there was no significant difference between right and left site location. Simpson grade I excision was performed in 4 cases (17%), Simpson grade II in 14 cases (58%), Simpson grade III in 5 cases (21%) and Simpson grade IV in one case (4%). The most common postoperative complication was wound site infection in 5 cases (21%). Postoperative tumor recurrence developed in 4 cases (17%). Conclusion: The prognosis of atypical meningiomas is relatively worse than meningiomas. The cases under the age of 50, total surgery excision and absence of increased intracranial pressure findings are positive prognostic factors. Risk of postoperative tumor recurrence is more than the other group of meningiomas even if it was excised as Simpson Grade I criteria. Total surgical excision as much as possible and post-radiotherapy is considered as an appropriate in terms of survival extension. Adjuvant RT reduces tumor recurrence, especially after incomplete surgery.

Giriş: Atipik meningiomlar tüm meningiom olgularının %20’sini oluşturmaktadır. Bu çalışmanın amacı, kliniğimizde opere edilerek atipik meningiom tanısı alan 24 hastayı retrospektif incelemektir. Gereç ve Yöntem:İzmir Bozyaka Eğitim ve Araştırma Hastanesi Beyin ve Sinir Cerrahisi Kliniği’nde 2013-2017 yılları arasında opere edilerek atipik meningiom tanısı alan 24 hasta çalışmamıza dahil edilerek yaş, cinsiyet, başvuru şikayeti, tümör lokalizasyonu, Simpson rezeksiyon derecesi, postoperatif komplikasyon ve nüks açısından değerlendirilmiştir. Bulgular:İntrakraniyal kitle nedeniyle kliniğimizde opere edilen ve meningiom tanısı alan 98 olgunun 24 tanesi (%25) atipik meningiom tanısı almıştır. Olguların 12’si erkek, 12’si kadındı. Yaş aralığı 24-71 arasında olup, yaşortalaması 55,87 idi. En sık başvuru şikayetleri, 16 olguda (%67) artmış intrakraniyal basınca bağlı semptomlar ve 7 olguda (%29) nörolojik semptomlardı. Sıklıkla konveksite ve parasagital/ falks bölge (%63) yerleşimli olup, sağve sol taraf yerleşim arasında anlamlı fark bulunmamıştır. 4 hasta (%17) Simpson derece I, 14 hasta (%58) Simpson derece II, 5 hasta (%21) Simpson derece III ve 1 hasta (%4) Simpson derece IV rezeksiyon yapıldı. Postop en sık komplikasyon yara yeri enfeksiyonu olarak 5 hastada (%21) görüldü. Postoperatif dönemde takiplerde 4 hastada (%17) nüks görüldü. Sonuç: Atipik meningiomlar prognozu nisbeten daha kötü meningiomlardır. 50 yaş altı olgular, total cerrahi eksizyon ve artmış kafa içi basınç bulgularının olmaması olumlu prognostik faktörlerdir. Simpson Derece I kriterlerine göre eksizyonunda bile diğer meningiomlara göre daha fazla nüks görülür. Mümkün olduğunca total cerrahi eksizyon ve sonrası radyoterapi sağkalım uzatılması açısından uygun yaklaşım olarak değerlendirilmek-tedir. Adjuvant RT özellikle subtotal cerrahi uygulanan olgularda tümör rekürrensini azaltır.
