Comparison of Intraocular Pressure Values Measured with Non-contact Tonometer and Pneumotonometer in Normal Individuals with Different Central Corneal Thicknesses

Abstract
Purpose: To compare intraocular pressure (IOP) values measured using non-contact tonometer (NCT) and ocular blood flow (OBF)-pneumotonometer in eyes with varying central corneal thicknesses (CCT). Materials and Methods: 130 eyes of 65 cases were included in the study. CCT values were measured for each eye, and IOP values were then measured using NCT and OBF-pneumotonometer devices. IOP measurements obtained with both devices were compared statistically. Results: Mean CCT value of the eyes in the study was 524.08±33.95 µ (433-607). Mean IOP values measured using NCT and OBFpneumotonometer were 15.57±3.25 (9.1-22.4) and 17.77±3.73 (9-23.9) mmHg, respectively. Although the differences in IOP measurements between the devices were statistically significant (p<0.001), the two devices exhibited a high degree of correlation with one another (r=0.8, p<0.001) and at Bland-Altman analysis the differences in IOP values were generally within±2SD. In addition, IOP values measured using both devices were affected by CCT values (r=0.385, p<0.001; r=0.435, p<0.001, respectively). For each 10-micron increase in CCT values, an increase of 0.37 mmHg for IOP values measured using NCT and a 0.48 mmHg increase for IOP values measured using OBF-pneumotonometer were observed. Conclusion: Results from NCT and OBF-pneumotonometer are proportional to one another. The OBF-pneumotonometer generally produces higher IOP values than NCT. However, this variation is not affected by CCT values. Interchangeable use of the two devices is not recommended in clinical practice. It is important that CCT values be borne in mind in assessing IOP values measured using the two devices.

Amaç: Non-kontakt tonometre (NKT) ve oküler kan akımı (OKA) pnömotonometresi ile ölçülen göz içi basıncı (GİB) değerlerini farklı merkezi kornea kalınlığı (MKK) değerlerine sahip gözlerde karşılaştırmak. Gereç ve Yöntem: Çalışmaya 65 olgunun 130 gözü katıldı. Her bir göz için MKK değerleri ölçüldükten sonra GİB değerleri sırasıyla NKT ve OKA-pnömotonometresi kullanılarak ölçüldü. Her iki cihazla elde edilen GİB ölçümleri istatistiksel olarak karşılaştırıldı. Bulgular: Çalışmadaki gözlerin ortalama MKK değerleri 524.08±33.95 (433-607) µ idi. NKT ve OKA-pnömotonometresi ile ölçülen ortalama GİB değerleri ise sırasıyla 15.57±3.25 (9.1-22.4) ve 17.77±3.73 (9-23.9) mmHg idi. Cihazlar arasındaki GİB ölçüm farklılıkları istatistiksel olarak anlamlı olsa da (p<0.001), her iki cihazın birbiriyle yüksek derecede korelasyon sergilediği (r=0.8, p<0.001) ve Bland-Altman analizinde iki cihazla elde edilen GİB değerlerine ait fark dağılımlarının çoğunlukla±2SD içinde olduğu görüldü. Ayrıca her iki cihazla ölçülen GİB değerlerinin de MKK değerlerinden etkilendiği tespit edildi (sırasıyla r=0.385, p<0.001; r=0.435, p<0.001). MKK değerlerindeki her 10 mikronluk artış için NKT ile ölçülen GİB değerleri için 0.37 mmHg, OKA-pnömotonometresi ile elde edilen GİB değerleri için 0.48 mmHglik bir artışın geliştiği görüldü. Sonuç: NKT ve OKA-pnömotonometresi birbirleriyle orantılı sonuçlar vermektedir. OKA-pnömotonometresi cihazı genel olarak GİB de- ğerlerini NKTden daha yüksek ölçmektedir. Ancak bu farklılık MKK değerlerinden etkilenmemektedir. Klinik uygulamalarda iki cihazın birbirlerinin yerine uygulanması uygun değildir. İki cihazla ölçülen GİB değerlerinin yorumlanmasında MKK değerlerinin göz önüne alınması önemli bir husustur.
