Histopathological profile of kidney biopsies: Single center, four years experience

Abstract
Objective: The studies investigating epidemiologic, clinical and histopathological findings related to renal disease are scarce in Turkey. This study aims to examine the histopathological profile of the renal biopsies evaluated at the Pathology Department of Izmir Tepecik Research And Training hospital. Methods: In this retrospective descriptive study, the archival files of the 506 biopsies performed between 2014-2018 at a single center were assessed. All biopsies, which were considered as adequate for pathological evaluation were subjected to direct immune fluorescent (DIF), and light microscopic examinations. Results: Male patients were the majority (n=294, 58.1%) of the materials. Sixty percent (n=307) of the biopsy materials were obtained from native and 39.3% (n=199) of them from transplant kidneys. In this series, 34.8% (n=176) of the cases were pediatric and 65.2% (n=330) of them were adult patients. The most common pathologies were nonspecific changes (n=33, 38.8%) and IgA nephritis (n=24, 28.2%) in children; while they were membranous glomerulonephritis (n=35, 15.8%) and nonspecific changes (n=28, 12.6%) in adults. The patients aged between 0 and 81 years, and the median age was 33±19.2 years. The median ages of the pediatric, adult patients were12.8±4.2 and 43.8±15.1 years respectively. Conclusion: In this study, nephrotic syndrome, proteinuria and hematuria were the leading clinical indications for native biopsies. Transplantation materials were more common in adults and males, and transplant patients were often referred to as protocol biopsies. Since no electron microscopy was performed, nondetectable FSGS or MLH patients were included in the nonspecific findings category. Since most of the children with MLH do not undergo a biopsy, it seems very difficult to obtain accurate information about their true incidence. For achieving better clinicopathological correlation, electron microscope evaluation, and genetic analysis in largescale studies are required to demonstrate the incidence of glomerulonephritis in our country.

Amaç: Türkiye’de böbrek hastalıkları ile ilgili epidemiyolojik, klinik ve histopatolojik araştırmalar çok sınırlıdır. Bu çalışma İzmir Tepecik Eğitim ve Araştırma Hastanesi Patoloji Laboratuvarında değerlendirilen böbrek biyopsilerinin histopatolojik profilini incelemeyi amaçlamıştır. Yöntem: Bu retrospektif tanımlayıcı araştırmada, tek merkezde 2014- 2018 yıllarında yapılan 506 böbrek biyopsisi arşiv bulguları üzerinden değerlendirildi. Patolojik değerlendirme için yeterli olarak kabul edilen olguların direkt immünfloresan (DIF) ve ışık mikroskopik düzeyde incelemeleri temel alındı. Bulgular: Erkek hastalar çoğunluktaydı (n=294, %58,1). Materyallerin %60,7’si (n=307) nativ, %39,3’ü (n=199) transplant biyopsi özelliğindeydi. Olguların %34,8’i (n=176) çocuk hasta, %65,2’si (n=330) erişkin hasta idi. Çocuklarda en sık izlenen patolojiler nonspesifik değişiklikler (n=33, %38,8) ve IgA nefriti (n=24, %28,2) iken, erişkinde membranöz glomerülonefrit (n=35, %15,8) ve nonspesifik değişiklikler (n=28, %12,6) idi. Hastalar 0 ile 81 yaş arasında olup, ortalama yaş 33±19,2 bulundu. Çocuk hastalarda ortalama yaş 12.8±4,2 iken, erişkin hastaların ortalaması 43.8±15,1 idi. Sonuç: Bu çalışmada, nefrotik sendrom, proteinüri ve hematüri, nativ biyopsilerin önde gelen klinik belirtileriydi. Transplantasyon materyalleri erişkinlerde ve erkeklerde daha yaygındı, transplantasyon hastaları sıklıkla protokol biyopsileri olarak gönderildi. Elektron mikroskobisi yapılmadığı için saptanamayan FSGS veya MLH hastaları, spesifik olmayan bulgu kategorisine dahil edildi. MLH’li hastaların çoğunda biyopsi yapılmadığından gerçek insidansları hakkında doğru bilgi elde etmek çok zor gözükmektedir. Ülkemizde glomerulonefrit insidansını göstermek için daha iyi klinikopatolojik korelasyon, elektron mikroskobu değerlendirmesi ve geniş ölçekli çalışmalarda genetik analiz yapılması gerekmektedir.
