Adölesan ve genç erişkin bireylerde bordetella pertussis seroprevalansının belirlenmesi

Abstract
i: 4/ es t arı . Objective: Whooping cough, caused by Bordetella pertussis, which can affect all age groups, is a conta- gious respiratory tract infection. After 4-6 years, due to decreased protection against the vaccine within 10 years, the average level drops too low. Age-specific pertussis seroprevalance data are used to determine whether booster doses of pertussis are required and if so, at what age it would be best to implement booster doses. The purpose of this study was to determine the seroeprevalence of pertussis in our region, and whether there is a need for rapel dose for pertussis vaccine or not based on the results obtained, and if there is a need make comments on the subject about when to carry this out. Material and Methods: Between January 2009 and January 2010, adolescent subjects were measured to determine the serum titre of IgG B. pertussis collect- ed, ranging from ages 13 to 30 (460 volunteer) young adult of subjects from B. pertussis antibody levels against pertussis antigens. Results: B. pertussis IgG antibody values were posi- tive in 81.09% (n=373), negative in 18.91% (n=87), for borderline 0.43% (n=2) of all participants. In the 13-18 group, 81.72% seropositivity (n=152), and in the 19-30 group, 80.66% seropositivity (n=221) was detected in 152, the relationship between age groups was not statistically significant (p=0.402). Conclusion: As adults lose their immune protection gained by pertussis vaccination, they become an infection source for infants. Therefore, in the current vaccination schedule, pre-school children and the children between 14-18 years, a booster dose of acellular pertussis vaccine would be useful.

Amaç: Boğmaca, Bordetella pertussisin neden oldu- ğu, tüm yaş gruplarını etkileyebilen, bulaşıcı bir solu- num yolu enfeksiyonudur. Aşıya bağlı koruyuculuğun 4-6 yıldan sonra azalması ve ortalama 10 yıl içinde çok düşük düzeye inmesi nedeniyle, gelişmiş ülkelerin aşı şemasına boğmaca aşı pekiştirme dozları eklenmiştir. Boğmaca-yaş spesifik seroprevalansının saptanması, pekiştirme dozlarının gerekli olup olmadığını ve gerek- li ise hangi yaşta uygulanmasının doğru olacağını saptamak için en değerli bulgulardan biridir. Bu çalış- ma bölgemizde genç erişkin bireylerde boğmaca seroprevalansını belirlemek, uygulanan boğmaca aşı takvimine ek rapel dozuna ihtiyacı belirlemek, rapel doz gerekliliği ve hangi yaşta uygulanması konusunda öneriler sunmak amacı ile planlanmıştır. Gereç ve Yöntemler: Ocak 2009-2010 yılları arası, yaşları 13 ile 30 arasında (460 gönüllü) genç erişkin kişilerden toplanan serumda B. pertussis IgG antikor düzeyi ELISA yöntemi ile analiz edildi. Bulgular: Katılımcıların %81,09unda (n=373) B. per- tussis IgG antikorları pozitif, %0,43ünde (n=2) sınırda değerde IgG antikor düzeyi belirlenirken, %18,91inde (n=87) IgG antikor düzeyi negatif olarak belirlenmiştir. 13-18 grubunda seropozitivite %81,72 (n=152), 19-30 grubunda seropozitivite %80,66 (n=221) olarak tespit edilmiş olup yaş grupları arasındaki ilişki istatistiksel olarak anlamlı bulunmamıştır (p=0,402). Sonuç: Boğmaca aşısı ile elde edilen koruyuculuğun yaşla giderek azaldığı belirlenmiştir. Bu nedenle mev- cut aşı takviminde okul öncesi ve 14-18 yaş arasında asellüler boğmaca aşısı ile pekiştirme dozu uygulan- masının planlanmasının faydalı olacağı inancındayız.
