Bullous hemorrhagic dermatosis associated with low-molecular weight heparin: A case report

Abstract

Currently, low-molecular weight heparins (LMWHs) are frequently used for the prophylaxis and treatment of pulmonary thromboembolism (PTE). LMWH shows its anticoagulatory effect through the activation of antitrombin III and the inactivation of Factor Xa. Its major side/adverse effects are bleeding, heparin-induced thrombocytopenia, hepatotoxicity, osteoporosis and skin reactions. In comparison to the more common skin reactions such as urticeria, itching, dermatitis, erythema; its rarely seen adverse effect is bullous hemorrhagic dermatosis. We found it appropriate to present our case with this rare adverse effect of LMWHs.

Özet

Günümüzde düşük molekül ağırlıklı heparinler, pulmoner tromboemboli (PTE) profilaksisi ve tedavisinde sıkça kullanılmaktadır. DMAH ise antitrombin III aktivasyonu ile Faktör- Xa’ yı inaktive ederek antikoagülan etkisini gösterir. Başlıca yan etkileri; kanama komplikasyonları, heparin-induced trombositopeni, karaciğer toksisitesi, osteoporoz ve cilt reaksiyonlarıdır. DMAH’ lerin ürtiker, kaşıntı, egzema, eritem gibi cilt yan etkilerine göre çok daha nadir görülen bir yan etkisi de bülloz hemorajik dermatozdur. DMAH’ lerin bu nadir yan etkisini gördüğümüz olgumuzu sunmayı uygun bulduk.
