Abstract

Although Brucella infection is a systemic disease, the microorganism may also cause local infections in the gastrointestinal system such as peritonitis, cholecystitis and pancreatitis. This study aimed at discussing, with the relevant literature, the brucella species which can cause gastro-intestinal involvement. Herein, a seronegative case of peritonitis due to Brucella spp is reported. A 48-year-old male patient was admitted to the emergency service with complaints of abdominal pain and abdominal swelling. The patient had hypertension, type-2 diabetes mellitus and chronic renal failure. He had been taking peritoneal dialysis for three years. Patient history revealed that the patient kept animals and had joint pain and fever for 1 year. The patient pre-diagnosed as having peritonitis by abdominal ultrasonography was admitted to the nephrology department. Peritoneum fluid cultures and blood cultures were taken on the first day of hospitalization. Brucella agglutination test was performed in the peritoneum fluid and found to be positive. However, the agglutination test in sera was negative. Brucellosis was diagnosed by serology and was confirmed by peritoneum fluid culture. Doxycycline 100 mg/day 2x1 and rifampicin 900 mg/day was given to the patient. A week later, there was progressive improvement in the clinical situation of the patient with treatment. Seronegatif Brusella PeritonitiOlgumuzda Brusellaya bağlı seronegatif bir peritonit olgusu sunduk. Kırk sekiz yaşında, karın ağrısı ve karında şişliği olan erkek hasta peritonit tanısıyla hastaneye yatırıldı. Hastanın hipertansiyonu, tip 2 diyabeti ve kronik böbrek yetmezliği vardı. Üç yıldır periton diyalizi yapılıyordu. Ayrıca hastanın anamnezinde evinde hayvan besleme ve bir yıldır geçmeyen eklem ağrısı ve ateşi vardı. Hastanın yapılan fizik muayenesi ve abdominal ultrasonografisi sonucunda peritonit ön tanısı ile nefroloji bölümüne yatırıldı. Yatışının ilk gününde periton sıvı kültürü ve kan kültürleri alındı. Periton sıvısından yapılan Brusella tüp aglütinasyon testi pozitif bulundu. Serumdan yapılan aglütinasyon testi negatifti. Periton sıvı kültüründen Brusella spp. identifiye edildi. Olgu, doksisiklin 100 mg/gün 2 x 1 ve rifampisin 900 mg/gün kullanılarak tedaviye başlandı. Bir hafta sonra hastanın kliniğinde belirgin düzelme oldu ve hasta taburcu edildi
