Evaluation of Gait and Balance in Epileptics with Reduced Bone Mineral Density

Abstract
Objectives: Bone mineral density abnormalities can cause falls due to imbalance and movement limitation. Morbidities that result from a fall mean a loss of functionality and quality of life. The aim of this study was to evaluate the relationship between bone mineral density and gait, balance, and fear of a fall in epileptic patients. Methods: A total of 104 patients were separated into 3 groups according to T-score. A metabolic serum profile analysis was performed, and the Tinetti Gait and Balance Scale (TGBS), the Falls Efficacy Scale (FES), the Short Cognitive Examination (SCE), the Beck Depression Scale, and the Neurocom Balance Master tests were administered. Results: In the osteoporosis group, the duration of medication use and the FES scores were high, the TGBS score and level of vitamin D were low, eyes-closed balance was more affected, and the degree of sway and turning to the left was greater, and tandem-walk speed was reduced. Positive correlations were found between age of onset and the TGBS and SCC scores, eyes-closed balance and duration of medication use, and FES score and eyes-open balance. Negative correlations were determined between the number of drugs used and the FES, TGBS, and CCM scores, as well as tandem-walk speed. Conclusion: Simple clinical tests can be administered in outpatient clinics to address the risk of falling due to gait and balance abnormalities in osteoporotic epileptics. Prevention can improve patient quality of life.

Amaç: Kemik mineral yoğunluğu anormallikleri, dengenin bozulması ve hareket yeteneğinin kısıtlanması sonucu düşmeye neden olmaktadır. Düşme sonucu ortaya çıkan morbiditeler ise bireyin fonksiyonelliği ve yaşam kalitesinde kayıp yaratmaktadır. Bu çalışmada epilepsili bireylerde kemik mineral yoğunluğunun; yürüme, denge ve düşme korkusu ile ilişkisinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Epilepsi polikliniğimizde takipli 104 hasta T skoruna göre; normal, osteopeni, osteoporoz şeklinde üç gruba ayrıldı. Hastaların parathormon, kalsiyum, fosfor, vitamin-D, vitamin-B12 ve folikasit düzeyleri değerlendirildi; Tinetti Yürüme ve Denge Testi (TYDT), Düşme Etkinlik Ölçeği (DEÖ), Kısa Kognitif Muayene, Beck Depresyon Ölçeği ve Neurocom Balance Master cihazında testler uygulandı. Bulgular: Osteoporoz grubunda; ilaç kullanım süresi ve DEÖ skorunun yüksek, TYDT skorları ve vitamin-D düzeyinin düşük; sünger üzerinde göz kapalı dengenin daha bozuk olduğu ve sola dönüş salınım derecesinin arttığı; bitişik adım yürüme hızının azaldığı görüldü. Pearson analizinde hastalık başlangıç yaşı ile TYDT ve KKM skoru; ilaç kullanım süresi ile süngerde göz kapalı denge; DEÖ skoru ile göz açık denge arasında pozitif korelasyon; ilaç sayısı ve DEÖ skoru ile TYDT, KKM skoru ve bitişik adım yürüme hızı arasında negatif korelasyon saptandı. Sonuç: Çalışmamız osteoporotik epilepsili bireylerde yürüme ve denge bozukluğuna bağlı düşme riskinin poliklinik şartlarında yapılabilecek basit klinik testlerle ortaya konabileceğini ve bu doğrultuda alınacak önlemlerle hastalarımızın yaşam kalitesinin yükseltilebileceğini göstermesi açısından önemlidir.
