"Ask the Addict about the Pleasure of Opium": The History of Drug Addiction in the Ottoman and Early Republican Periods

Abstract
The subject of this work is the history of narcotic substance abuse and substance abusers in the Ottoman and Early Republican periods. The point of agreement of both those who are termed "addicts" and those who are opposed to the use of such narcotics is the belief that the use of these substances gives the user feelings of "pleasure" and "a high." Thus, for many years, the related literature in Turkish referred to such substances as "pleasure-giving substances." This pleasure-seeking culture revolved around such narcotics as opium, hashish, morphine, heroin, and cocaine. My aim in this study is to demonstrate that the topics related to these narcotics and their users are not only of interest to the realm of medicine, but are also important for their historical perspectives. However, we must also clarify that such a historical investigation does not imply that this work will provide a chronological and static recap of the long history of these narcotic substances. While at the beginning of the 19th century the use of these substances was viewed from the perspective of ethics and morals, by the end of the century, it had become the subject of medicine. Despite this, this medical perspective was still nourished by generally espoused beliefs of ethical and moral correctness. As a matter of fact, Republican era physicians had inherited the strong Ottoman aspiration to establish a healthy population. The underlying reason for their concerns about substance addiction and their wish to treat this was their belief that such addiction was a "disease of immorality," and the end results of this disease would be that of being sent to prison, mental institutions, or death for its victims.
Özet
Bu çalışmanın konusu, Osmanlı İmparatorluğu ve Erken Cumhuriyet Türkiyesi'nde uyuşturucu madde bağımlılığı ve bağımlılarının tarihidir. Gerek bağımlılar gerekse karşıtları-nın üzerinde anlaştıkları nokta, madde kullanımının kişiye büyük bir "haz, keyif" verdiğidir. Bu özelliği ile Türkçe literatürde keyif verici madde ifadesi daha eski ve daha uzun süre kullanılmıştır. Yazı, keyif kültürünün tüketim maddeleri olarak afyon, esrar, morfin, eroin ve kokain üzerinde durmaktadır. Amacımız bu maddeler ve kullanıcılarının yalnız tıbbın değil tarihin de konusu olabileceğini göstermektir. Fakat hemen belirtelim ki, bir tarih çalışması olması maddelerin uzun tarihinin kronolojik ve statik bir tekrarını yapmak anlamına gelmez. Daha 19. yüzyıl başında madde kullanımı ahlakî bir mesele iken yüzyılın sonlarında tıbbın konusu haline gelmişti. Fakat tıbbî duyarlılığı da ahlakî algı ve kanaatler besledi. Nitekim Osmanlı'daki sağlıklı bir toplum kurma isteği Cumhuriyet Türkiyesi'nin doktorlarına güçlü bir miras olarak kalmıştı. Onların madde bağımlılığına dair kaygılarının ardındaki sebep, bağımlıları öncelikle tedavi edilmeleri gereken "ahlâk hastaları" olarak görmelerindendi. Bu öyle bir hastalıktı ki, kurbanlarının sonu hapishane, tımarhane veya mezardı.
